About the role
The Senior Team Leader provides clinical leadership and governance across VMCH’s At‑Home Aged Care services, including Support at Home (SAH) and CHSP programs. This role oversees restorative care, end‑of‑life and social work pathways, providing supervision and support to a multidisciplinary team to ensure safe, high‑quality, person‑centred care.
Working closely with internal teams and external partners, the Senior Team Leader ensures compliance with Aged Care Quality Standards, supports complex clinical decision‑making, and drives consistent, high‑quality service delivery across regions.
Step into a role where you will:
- Provide clinical leadership, supervision and governance across restorative care, end‑of‑life, short‑term pathways and social work services, supporting ethical decision‑making in complex cases.
- Lead and supervise a multidisciplinary team, facilitating MDT meetings and ensuring consistent, high‑quality clinical practice.
- Oversee clinical assessments, care planning, participant budgets, risk management and quality outcomes, including incidents, complaints and audits.
- Ensure compliance with Aged Care Quality Standards, SAH legislation, OH&S and VMCH policies, while building strong internal and external stakeholder relationships and representing VMCH in sector forums.
About you
You are an experienced clinical leader who thrives in complex service environments and is passionate about supporting people to remain living independently in the community. A collaborative leadership approach, sound governance capability and a strong commitment to quality and safety underpin success in this role.
You will bring:
- Tertiary qualification in Allied Health or Social Science
- Current AHPRA registration (or relevant Allied Health registration)
- Proven experience in case management with complex client needs
- Demonstrated experience leading staff, managing KPIs and supporting performance
- Strong assessment, care planning and budget management skills
- Sound knowledge of aged care reforms, quality standards and community care legislation
- Excellent communication, time management and stakeholder engagement skills
- Willingness to travel regionally
- Current Victorian Driver’s Licence and National Police Check clearance
