About the role
VMCH is seeking a passionate Community Occupational Therapist to join our At Home Aged Care (AHAC) Allied Health team on a 12‑month fixed‑term maternity leave contract. This is a rewarding, community‑based role working closely with older clients across Melbourne’s aged care planning regions.
In this role, you will provide high‑quality occupational therapy assessments and interventions to older clients living at home and accessing services through CHSP, Home Care Packages and other funding programs.

- Conduct comprehensive assessments and create tailored interventions
- Collaborate with clients to set goals and develop care plans
- Deliver group programs focused on cognition, balance, wellness, and reablement
- Work closely with a multidisciplinary team including nurses, occupational therapists, and care coordinators
- Maintain accurate documentation and meet billable hour targets
- Travel across aged care planning regions as required
About you
- A minimum of one year’s experience working as a full‑time Occupational Therapist
- Deliver comprehensive community‑based OT assessments and interventions aligned to Aged Care Quality Standards
- Recommend assistive technology, equipment and home modification solutions
- Complete high‑quality reports, documentation and referrals
- Collaborate with My Aged Care, RAS, GPs, Case Managers and funding bodies
- Deliver individual and group‑based OT programs
- Manage an autonomous caseload, meet achievable billable targets and travel across aged care regions as required
