Job Description:

As a Clinical Case Manager II, you will work under the supervision of the Medical Directors of the NAVIGATE Program at the UF Health Springhill Clinics. This position is one of a team of NAVIGATE Program professionals providing clinical interventions focused on coordinated specialty care for patients experiencing first episode of non-affective psychosis. Clinical duties including triaging with patients and their families using specialized training from the NAVIGATE program. Regular NAVIGATE team meetings are an important part of the clinical duties of this position. This position will conduct clinical interviews to identify vocation and educational goals of patients within the program and provide ongoing developmental surveillance of NAVIGATE patients and families support needs as a Supported Employment and Education Specialist (SEES).

Case Management and Clinical Support Services – Provide comprehensive case management and clinical support for NAVIGATE patients and families. Documents NAVIGATE team evaluations and treatment plans, working collaboratively with the NAVIGATE team to develop and implement approved treatment plans using professional judgement and discretion Coordinates direct patient therapy sessions, including individual resiliency and family education sessions, and provides services when clinical coordinators are unavailable. Ensures NAVIGATE data elements are captured in real time across all cases. Supports patients in achieving education and employment goals by providing counseling, identifying resources, and assisting with tasks such as resumes and other return‑to‑work or school activities. Participates in weekly team meetings to provide updates on patient progress, needs, and challenges, and collaborates with peer support specialists to advocate for patient needs.

Program Support – Work closely with both NAVIGATE program leadership and team members to support patient care and program operations. Assists with scheduling, adding patients according to program protocols, facilitating patient outreach, and ensuring accurate follow‑up documentation as required by the funding agency. Helps patients access housing, transportation, and other community resources. Compiles patient lists for weekly case conferences, advocates for patient and caregiver needs, and ensures timely follow‑up. Participates in program networking events, identifies opportunities to strengthen partnerships, and develops relationships with schools and community organizations to enhance patient support.


Community Outreach - Provides consultation, education, and support to individuals and families diagnosed with first‑episode non‑affective psychosis (FEP). Collaborates with schools, agencies, and community partners to increase awareness and acceptance of individuals experiencing FEP. Engages community resources to connect families with needed services. Writes consultation notes, completes required reports, and maintains all documentation in a timely manner. Develops educational materials for community members, educators, and families. Collaborates with agencies across the State of Florida to remain current on clinical developments and ensure consistent delivery of NAVIGATE services.


Program Reporting – Track, compile and develop protocols to provide reporting needs for the NAVIGATE program’s funding agency. Document and track consultations and crisis management activities for the reporting agency.

Minimum Qualifications: 

Master’s degree in an appropriate area of specialization and two years of experience; or a bachelor’s degree in an appropriate area of specialization and five years of appropriate experience.

Preferred Qualifications:
  • Experience with schizophrenia and first episode psychosis
  • Experience with behavioral assessments and providing interventional therapy
  • One year of clinical consulting experience in helping individuals in psychiatric care programs
  • Experience serving individuals enrolled in SSI, Medicaid, and Medicare programs, along with a working
  • knowledge of community-based services, social support systems, and resource referrals.
  • Experience working with older youth and young adults, including the ability to build rapport, support
  • transition planning, and connect individuals to relevant educational, vocational, and community services.
  • A licensed social work credential (LSW/LMSW/LCSW)
Expected Salary:

$62,000 - $70,000
