This position will support both therapy services and program coordination for the Diabetes STEPs Program.

This position will provide strategic leadership and operational oversight for statewide diabetes prevention, education, outreach, and management initiatives. The role will lead statewide diabetes screening and outreach efforts, oversee technology-enabled education and disease management programs, and support data collection, program evaluation, and research initiatives to improve diabetes outcomes and expand access to care across Florida.

In addition to program leadership responsibilities, the coordinator serves as a clinical social worker within the endocrinology clinic, providing psychosocial assessment, evidence-based psychotherapy, and behavioral health interventions for individuals and families affected by diabetes and other endocrine disorders. Working as part of an interdisciplinary healthcare team, this role addresses the emotional, behavioral, and social factors that influence disease management, treatment adherence, and quality of life. The position integrates direct patient care with program development to ensure that diabetes services are comprehensive, patient-centered, and responsive to the needs of various populations throughout the state.

Serves as a liaison between the University of Florida, healthcare systems, community organizations, state agencies, and other key stakeholders to advance statewide diabetes prevention, education, outreach, and care initiatives. This position requires expertise in program leadership and coordination, partnership development, community engagement, behavioral health service delivery, and interdisciplinary collaboration to support both population health efforts and patient-centered diabetes care.

Direct Patient Care and Behavioral Health Services:

Provide psychosocial assessment, evidence-based psychotherapy, and behavioral health interventions for patients with diabetes and other endocrine disorders within the clinical setting. Collaborate with endocrinologists, diabetes educators, and other members of the interdisciplinary care team to address psychosocial barriers to disease management, improve treatment adherence, and support patient self-management and quality of life.

Clinical Program Coordination and Administration:

Coordinate and support implementation of the Diabetes STEPS Program and related clinical initiatives. Assist in developing, implementing, and evaluating program policies, procedures, and workflows to improve patient care, program efficiency, and service delivery. Serve as a liaison among University of Florida faculty, healthcare providers, researchers, community organizations, and state stakeholders to facilitate communication, collaboration, and achievement of program objectives. Coordinate program activities, maintain documentation, and assist with reporting requirements to support successful program operations and statewide impact. Incumbent will supervise Peer Coaches and volunteers on the State of Florida Type 1 Diabetes Screening Program. This will include travel to multiple clinic sites across the assigned region and community level outreach.

Outreach, Education, and Program Development:

Coordinate diabetes education, screening, outreach, and community engagement activities in collaboration with University of Florida faculty, healthcare systems, community organizations, and state agencies. Develop and maintain partnerships to support program growth and expand access to diabetes prevention, education, and management services. Provide health education and consultation to patients, healthcare professionals, and community partners regarding diabetes management and available resources.

Evaluation, Data Management, and Research Support:

Support program evaluation, quality improvement, and research activities related to diabetes prevention and management initiatives. Collect, manage, analyze, and report program metrics and outcomes to support strategic planning, funding requirements, and stakeholder reporting. Assist with diabetes tissue biobanking operations and coordination of related research activities. Collaborate with clinical, research, and administrative teams to ensure accurate data collection, regulatory compliance, and effective dissemination of program outcomes to university leadership, funding agencies, and community partners. Work to establish and enhance state-wide Type 1 Diabetes screening protocols and processes.

$85,000 - $95,000 annually; commensurate within this range based on qualifications and experience

Bachelor's degree in an appropriate area and four years of relevant experience; or an equivalent combination of education and experience.

• Master's degree in Social Work (MSW) from a program accredited by the Council on Social Work Education (CSWE) and eligibility for licensure in the State of Florida or Master’s degree in psychology, mental health counseling, or other related field and two years post-degree supervisor experience.
• Demonstrated leadership, organizational, and communication skills.
• Experience providing psychosocial assessment, counseling, and behavioral health interventions to individuals, families, and groups. Knowledge of evidence-based therapeutic approaches, crisis intervention, case management, and interdisciplinary healthcare collaboration. 
• Experience in diabetes prevention, management, or research.
• Familiarity with continuous glucose monitoring (CGM), health technology applications, or artificial intelligence in healthcare. 
• Proven success in managing large-scale, multi-stakeholder initiatives and budgets.
• Track record of partnership development and cross-sector collaboration.

This is a time-limited position.

Normal work hours are Monday through Friday, 8:00 a.m. to 5:00 p.m.
