Functions as a member of a multidisciplinary team organized to provide and coordinate services for victims of child abuse and neglect and their families. The position requires knowledge in the areas of child abuse and neglect and family dynamics, skills in the areas of assessing/case management, crisis intervention, and making appropriate referrals. Based in in Jacksonville, FL, this position includes the catchment area of Baker, Clay, Duval, Nassau, Putnam, St. Johns, Volusia and Flagler counties.
Receives referral information during agency business hours, which are received by the Department of Children and Families, Law Enforcement and other referral sources, in the eight (8) Northeast Florida Counties covered by the UF First Coast Child Protection Team. Ensures all necessary information is obtained and documented in CPTIS or on the appropriate Intake form as required. Provide all intake/referral forms to supervisor on daily basis for assignment. Staff all referrals with the CPT Medical Provider(s) and Supervisor(s) and schedules forensic medical exams, interviews and other appointments as needed. Notifies all community partners involved of appointments dates/times. Consults with therapists, psychologist, internal and external medical providers, hospitals, State Attorneys Offices and external community partners regarding case referrals. Reports problems with the referral process or an individual referral, to the Team Coordinator or Assistant Team Coordinator, within the required timeframe.
Organizes and performs office and clerical functions including, but not limited to, manages the front desk reception area, answers phones, transfers calls, ensures the lobby area is clean and organized, greets clients and obtains intake paperwork for client visits. Establishes and/or maintains filing systems and workflows, filing and tracking the records sent to the offsite records storage facility, sending information (interviews, documents, etc.) to the appropriate parties per statute guidelines. Provides general administrative support to the CPT First Coast Program and leadership.
Ensures that copies of intake exception reports are sent to the Department of Children and Families daily.
Participates in community training/outreach activities as directed by the Team Coordinator.
