About the Role
As a Care Coordinator you will oversee and enhance the delivery of our Aged Care services in Geelong. The successful candidate will be responsible for processing referrals, assisting clients in commencing services, and continuously reviewing and adjusting care plans to suit their evolving needs. You will coordinate home visits to ensure the needs of the people we support are met and that we are working in partnership with them and their families.
This is a permanent full-time role, based in our Geelong office with hybrid work from home arrangements and travel to clients’ homes in the Greater Geelong area, Surf Coast, Bellarine Peninsula through to Lara when required.
Key Responsibilities
- Undertake comprehensive client assessments while developing client profiles and care plans in collaboration with the client/representative and other relevant stakeholders
- Ensure care plans are monitored and reviewed on a regular basis to align with the changing needs of clients, including regular client home visits
- Understanding of clinical needs, health conditions, and care planning is essential, though formal clinical qualifications are not required.
- Collaborate effectively with internal stakeholders to ensure clients’ needs are met including our Rostering team and Practice Leads
- Develop strategies and/or broker services which will address the identified needs of Aboriginal and Torres Strait Islander clients in consultation with families and communities
- Represent Life Without Barriers at external forums and meetings, promoting the organisation within the community
Skills & Experience
- Demonstrated Aged Care or Allied Health case management experience with strong administration skills, and ability to work with individuals, families and/or stakeholders
- Understanding of Home Care Packages, Commonwealth Home Support Programme, Aged Care services and My Aged Care Standards
- High level competency in utilising a variety of technology including mobile devices and computers, proficiency with Microsoft Office, Outlook and database systems
- Ability to identify and implement risk mitigation strategies
- High level communication, influencing, liaison and negotiation skills and established problem solving and conflict management skills.
- Ability to multi-task and work autonomously in a fast-paced environment
- A current unrestricted Australian Driver Licence, reliable vehicle and a willingness to travel
