About the Role
The purpose of the Case Manager – Transition from Care (TFC) position is to provide outreach style case management support to young people between the ages of 15 and 18 who are transitioning from care to adulthood and independence.
The successful candidate will work in partnership with the young people, carers, relevant government agencies and stakeholders to develop skills and implement individualised care plans.
This position is being offered is full-time based at our office in Robina. A Tool of Trade vehicle (including fuel card) will also be allocated to the successful candidate.
Key Responsibilities
- Develop high quality case plans which respond to the individual and changing needs of children and young people in LWB’s care.
- Develop comprehensive plans for each young person 15 years and over to support their transition to independent living and ensure young people are supported to access financial and other support.
- Provide support, advice and referrals to young people.
- Collaborate with Cultural Support Planners to incorporate and implement strategies within case plans to support Aboriginal and Torres Strait Islander children and young people to maintain their culture and identity.
- Drive the implementation and achievement of the case plan goals and plans for permanency of children and young people.
- Record, monitor and review goals, objectives, and tasks to ensure their ongoing relevance to each child or young person.
- Ensure the CIRTS database is up to date with all client information.
- Manage communication with key stakeholders, internally and externally to achieve case plan goals.
Skills and Experience
- A relevant bachelor’s degree in Social Work, Social Welfare, Psychology, Nursing or Mental Health.
- A current driver’s licence.
- Demonstrated experience in case planning with children and young people who have experienced trauma with a focus on achieving permanency outcomes.
- Demonstrated experience in developing positive developmental relationships that build trust with children and young people through visible interest and active engagement, and to place the best interests of the child at the centre of case planning.
- Demonstrated experience in working autonomously in implementing therapeutic case plans for multiple clients simultaneously.
Successful candidates will be required to clear probity checks including National Criminal History Record Check and Working with Children Check (where relevant to the role).
