Role: Case Manager – Family, Based Care

In this role, you will provide psychological assessment, therapy, and behaviour support to children and young people in foster care, residential care and those accessing NDIS funded services. You will work directly with clients to address trauma, disability and developmental needs, adapting interventions to their unique circumstances.
You will also provide high level psychological advice and deliver structured, evidence-based treatment services to support the Child, Youth and Family Program, ensuring interventions are guided by best practice and tailored to the individual needs of each child.
About the role
We are recruiting a Case Manager to join our Family Based Care Program. We work closely with the Department of Families, Fairness and Housing to provide care arrangements either through Foster Care or a Targeted Care Package (TCP).
As a program we are responsible for ensuring that any child or young person placed in our care is provided with a safe and nurturing environment. We work closely with DFFH and other stakeholders to ensure that we are supporting the child in the context of their case plan and court order. As a case manager, you will be responsible for ensuring that the program requirements for home-based care are implemented into practice.
This position reports to the Operations Manager, is being offered as a full-time permanent position and will be based in Dandenong, they will also be required to travel across metro and regional areas at times.
Key Responsibilities
- Providing case management to children and young people in a range of different out of home care arrangements, predominately Foster care. Your caseload may also include supporting young people through a Targeted Care Package (TCP) where they could be living with parents, kinship carer or in independent living.
- Building positive relationships with children, young people and carers through regular home and outreach visits
- Ensuring that we are including the client voice in our care planning and preparing them for leaving care.
- Working as part of a care team to ensure the co-ordination of supports and services to ensure the child and young person’s health, development and wellbeing needs are being met.
- Providing regular support and coaching to foster carers to ensure they provide a safe and secure environment for the children and young people in their care.
Skills & Experience
- Bachelor level qualifications in Social Work, Social Science, Psychology or Human Services
- Knowledge of trauma informed care and responses to pain based behaviours.
- Experience providing case management to children and young people, preferably within the child protection context.
- Demonstrated ability to manage a caseload ranging in complexity and able to prioritise competing demands.
- Experience developing quality case plans or individual plans as well as leading the implementation of the goals within the plan.
- Experience working with individuals, families or groups from a culturally appropriate perspective.
- Demonstrated ability to communicate effectively with a range of stakeholders and being able to produce written reports and case notes to high standard.
- It is a requirement that you hold a current unrestricted driver’s licence.
Successful candidates will be required to clear probity checks including National Criminal History Record Check and Working with Children Check (where relevant to the role).
