Job description:

Care@home is a state-wide interdisciplinary service that remotely supports patients, to better self-manage their illnesses and chronic conditions in their own home using virtual care technologies, improving patient health outcomes, and reducing potentially preventable hospital admissions.

The Team:

Care@home is a state-wide interdisciplinary service that remotely supports patients, to better self-manage their illnesses and chronic conditions in their own home using virtual care technologies, improving patient health outcomes, and reducing potentially preventable hospital admissions.

This virtual care service has been established as a state-wide navigation hub, that provides a single point of access for home and community-based care; making it easier for both consumers and clinicians to identify what care pathways are available to support people in their preferred environment.

This interdisciplinary service employs General Practitioners who deliver virtual clinical consultations, support for GP Registrars and junior doctors and short-term primary care deployment to community settings where there are critical risks to the delivery of services.

The Role:

The Specialist Medical Practitioner – General Practitioner is responsible for:
  • Providing specialised virtual care to patients enrolled in THS Virtual Care Services, managing non urgent acute and chronic conditions suitable for management in a home care environment.
  • Delivering a full range of virtual clinical services, such as triage, assessment, patient management plans, clinical governance, guidance, advice, support, and expert opinions.
  • Support private General Practice facing significant staffing challenges when no alternative solutions are available including the provision of face to face and virtual care consultations.
  • Provide interim supervision to GP Registrars, International Medical Graduates (IMGs), and Junior Doctors working in District Hospitals and temporary deployment to primary care in high-risk community settings.

Details of appointment:

Three fixed term full time, day worker, working up to 76 hours per fortnight, commencing from as soon as possible for a period of two years

*notwithstanding hours to be negotiated with the successful applicant

Salary:

Specialist Medical Practitioner Level 1- Senior Specialist Medical Practitioner Level 3 $216,300 - $310,389 per annum + 11.5% superannuation + access to salary packaging

Please note that access to the Senior Specialist Medical Practitioner salary range $300,216 -$310,389 is subject to the successful applicant satisfying the criteria for Senior Specialist Medical Practitioner Level 1, 2 or 3 as outlined in the Medical Practitioners (Tasmanian State Service) Award

  • Salary range is in accordance with Medical Practitioners Agreement 2022

In addition, the following allowances and benefit, calculated on the salaried incremental point may be available:

  • Salary packaging options include a range of benefits like living expenses up to $9,010 and $2,650 in meals and entertainment every year.
  • Provision of a fully maintained private plated motor vehicle or motor vehicle allowance of $28,389 per annum.
  • Subject to approval from the Private Patient Scheme Committee (PPS), 35% of base salary of additional payment will be paid from (PPS). If not admitted to the PPS, a 35% in lieu of PPS of base salary will be paid.
  • Continuing Professional Development Allowance of $16,480 per annum
  • Travel and relocation assistance support available up to $15,000 for eligible appointees from overseas, and up to $10,000 for appointees from interstate.
  • Professional development and accelerated pathways.
  • A range of leave entitlements, including study leave and Professional Development Support.
  • Fitness Passport – You, and your family can access 40+ fitness facilities across Tasmania for just $13.95 per week per person, or $25.95 for a family.
  • other allowances as applicable.

Eligibility:

Eligibility: Successful applicants will be required to meet the essential criteria as

  • Holds specialist registration
  • Is a registered Medical Practitioner who is an International Medical Graduate (IMG) who is on the specialist pathway.
  • Is a registered Medical Practitioner who is an International Medical Graduate (IMG) who has a recognised overseas specialist qualification and is assessed as having sufficient experience in the speciality.
  • Current Working with Children Registration (where applicable and as determined by individual position requirements).


*Registration/licences that are essential requirements of this role must remain current and valid at all times whilst employed in this role and the status of these may be checked at any time during employment. It is the employee’s responsibility to ensure that registration/licences remain current and to advise the Employer if their circumstances change. This includes notifying the Employer if a registration/licence is revoked, cancelled or has its conditions altered.

  • Fellowship with the Royal Australian College of General Practitioners (FRACGP) or the Australian College of Rural and Remote Medicine (FACRRM)
