Job description:

Exciting opportunity for a Clinical Nurse Specialist to join the Department of Health!

The Team:

 Royal Hobart Hospital is Tasmania’s largest hospital, and the Aged Services Team (AST) is a specialist nursing service working in conjunction with the Aged Care Medical Team, providing a consultative service within the RHH. The AST represents and advocates for people with aged related conditions and their carers, promoting a culture of respect for the older person within the Tasmanian Health Service (THS). All our services work closely together to focus on delivering excellence in healthcare to achieve improved outcomes for our consumers, our workforce, and the Tasmanian community.

The Role:

  1. Undertake comprehensive geriatric assessments and coordinate referrals, to ensure timely assessment of patients and management of complex discharge planning. 
  2. Provide expert clinical specialist advice and case coordination, in consultation with members of the wider multidisciplinary team.
  3. Assists the Nurse Unit Manager (NUM) and other senior staff with the planning and management of staff
    and resources and the coordination of day-to-day client care activities and flow.
  4. Act as a resource for ward/allied health staff, for information and advice on Aged related diseases or
    conditions, participating in in-service education as required, providing leadership in contemporary nursing
    practice, ensuring risk identification and appropriate clinical care pathways.
  5. Assist in the development of relevant risk management strategies, in accordance with best practice and
    client focused outcomes, and contribute to the implementation of clinical service policies and clinical
    pathways to assist in meeting service objectives.
  6. Contribute to, and participate in, nursing research and quality improvement activities that will contribute to
    the formulation and evaluation of state-wide Aged Care policies including participating in AST data
    collection processes for evaluation and research purposes.
  7. Actively participate in and contribute to the organisation’s Quality & Safety and Work Health & Safety
    processes, including in the development and implementation of safety systems, improvement initiatives,
    safeguarding practices for vulnerable people, and related training. 
  8. The incumbent can expect to be allocated duties, not specifically mentioned in this document, that are
    within the capacity, qualifications and experience normally expected from persons occupying positions at
    this classification level.

You will need:

  1. Proven highly developed clinical experience in aged care issues and service delivery.
  2. Expertise, formal training and/or experience in comprehensive geriatric assessment and a range of geriatric syndromes including dementia and delirium assessment and management with the ability to develop, implement and evaluate comprehensive care plans and discharge pathways.
  3. Ability to function instructively as a member of a multidisciplinary team with the capacity to act in a leadership role, set realistic goals, evaluate work performance and provide own self-care.

Details of appointment:

Permanent, part time, day worker position, working 40 hours per fortnight, commencing as soon as possible. 

*notwithstanding hours to be negotiated with the successful applicant.

Applicants should note that, for a period of twelve months from the date of publication, this selection process may be used to fill subsequent or similar full time, part time, and casual vacancies.

Salary: $106,570 to $112,440 per annum (Pro rata). Our Employer 12% superannuation contribution is on top of this amount.

  • Salary range is in accordance with Nurses and Midwives (Tasmania State Service) Agreement 2023.

In addition, the following allowances calculated on the salaried incremental point may be available:

  • Post Graduate Allowance of up to 10 % based on qualification type

Eligibility:

Successful applicants will be required to meet the essential criteria.

  • Registered with the Nursing and Midwifery Board of Australia as a Registered Nurse

  • Current Driver’s Licence

Applicants should note the following criteria are desirable:

  • Postgraduate qualifications in Aged Care nursing or working towards a relevant postgraduate qualification or experience in the management of clients with aged care issues such as delirium and dementia
