Job description:

The Clinical Nurse Specialist - Aged Services Team (AST): • Provides discipline specific support and advice within the relevant Hospital multidisciplinary team, and offsite units. • Undertakes comprehensive geriatric assessments identifying early discharge issues and enabling the early implementation of discharge planning processes for complex Aged Care patients. • Works in collaboration with the NUM and other senior staff to provide clinical leadership to the relevant Hospital wards, Sub-Acute, Aged and Community services, to enable the delivery of quality patient care to aged care clients referred to the service.

The Team:

The Clinical Nurse Specialist for Aged care services assist the dynamic hospital aged care liaison (HALT) & Residential aged care home liaison (RACHL) team at the Launceston General Hospital. 

The Role:

  • Supporting Older Australians and their families within the Launceston General Hospital (LGH).
  • Supporting the Hospital Aged Care Liaison Team (HALT), Residential aged care home (RACH) Clinical Nurse Consultants and the Nursing unit Manager.
  • Proven highly developed clinical experience in aged care. 
  • Ability to function instructively as a member of a multidisciplinary team. 
  • Working with RACH residents within the LGH.

Details of appointment:

Fixed term part time, day worker position, working 64 hours per fortnight, commencing from a mutually agreed upon date until 31 October 2028. 

*notwithstanding hours to be negotiated with the successful applicant.

Applicants should note that, for a period of twelve months from the date of publication, this selection process may be used to fill subsequent or similar full time, part time, and casual vacancies.

Salary: $106,570 to $112,440 per annum (Pro rata). Our Employer 12% superannuation contribution is on top of this amount.

  • Salary range is in accordance with Nurses and Midwives (Tasmania State Service) Agreement 2023.

In addition, the following allowances calculated on the salaried incremental point may be available: 

  • Post Graduate Allowance of up to 10 % based on qualification type

Eligibility:

  • Registered with the Nursing and Midwifery Board of Australia as a Registered Nurse
