Job description:
Following Agency policies, primary health care principles, legal requirements and professional competencies, the Clinical Nurse Consultant - Community Health Nursing West Coast (CNC), in collaboration with the Nurse Unit Manager (NUM) and Director of Nursing (DON), provides clinical leadership and support to the community healthcare team by coordinating the provision of high quality community health nursing, and ancillary services to clients on the West Coast based on national standards and chronic disease best practice principles within a multidisciplinary framework.
In consultation with the NUM, proactively direct, lead, and participate in the development of an education and research culture. Support the NUM in developing, implementing, and evaluating clinical practice standards, including monitoring clinical competencies of the health care team, and actively participating in the professional development of individual team members.
As a senior member of the healthcare team, support the NUM in a broad range of administrative, service delivery, and business planning issues relating to the provision of community nursing, adult day centre, home, and community care.
