About the opportunity

The Home and Community Care Program For Younger People (HACC PYP) supports younger people with disabilities or medical/health conditions, and their respective carers to live as independently as possible in their own homes. Younger people are defined as people less than 65 years, or Aboriginal and Torres Strait Islander people aged less than 50 years. The Program assists with a range of daily activities including personal care, dressing, preparing meals, house cleaning, property maintenances, community access and using public transport.

As Team Leader, you will provide effective leadership, supervision and support to a dedicated team delivering HACC PYP services across the Baw Baw and Latrobe local government areas. The role will contribute to the ongoing development of high quality, client-focused services and ensure compliance with relevant legislation, regulations and standards. The Team Leader will also establish and maintain collaborative relationships with partner organisations and agencies across the broader service system.

About you

To be successful in your application you will meet the person specification in the position description, including having a tertiary qualification in Disability, Social Work, Health, Welfare, Community Services or a related discipline.

You will also have:

  • Previous experience in the provision of care to people with a disability, younger people, and support for their carers
  • Demonstrated experience in leading and developing a team within a complex service delivery environment
  • Demonstrated experience in performing community and/or health assessments using a strengths-based approach
  • Sound understanding of legislation and regulations relevant to the disability and community services sector
  • Proven ability to establish collaborative partnerships with other services, agencies and professionals to achieve optimal client outcomes
  • Excellent communication, problems solving and analytical skills
