Title:            Project Coordinator - Grant - Rural Health Transformation Project

Facility:         Rowan-Virtua School of Osteopathic Medicine

Summary:

Serve as Project Coordinator to implement and oversee grant activities, including the operation of a mobile health unit in Cumberland County, New Jersey.

Primary Duties/Responsibilities:

  • Work with the Project Director to implement the proposed activities in the grant. This includes, but is not limited to, developing goals and operational systems for the mobile unit.

  • Develop training materials on local resources for the Community Health Workers (CHW) and Rural Health Service Corps student members.

  • Train CHWs and Rural Health Service Corps student members.

  • Provide supervision to community health workers (CHWs) to screen Cumberland County residents for chronic health conditions and ensure they are linked to care.

  • Coordinate a Rural Health Service Corps of medical students who will assist in screening residents for chronic disease and implementing new interventions to support rural health.

  • Coordinate the implementation of new nutritional interventions.

  • Ensure data collection is implemented in accordance with grant reporting requirements.

  • Prepare and submit program reports under the supervision of the Project Director and Co-Director. 

  • Coordinate with project partners to ensure grant activities and timelines are being met and serve as a liaison between all entities.

  • Assist in creating project schedules that reflect project team involvement and determine the best course of action to ensure project goals are met.

  • Work with the Project Director to develop a calendar of outreach events and coordinate access to ample supplies and staffing for events.

  • Work with the Project Director and partners to coordinate community engagement and outreach.

  • Oversee interfacing with an electronic health record for data collection.

  • Coordinate Mobile Staff operations.

  • Participate in all training required for the grant.

  • Be available to serve as driver of mobile unit as needed

  • Attend meetings as required.

  • Assist in the preparation of reports and spreadsheets containing findings, analyses, conclusions, and recommendations as requested. Assist in the preparation of reports required by the sponsor. 

  • Understand and adhere to Rowan-Virtua SOM compliance standards as they appear in the Corporate Compliance Policy, Code of Conduct, and Conflict of Interest Policy

  • Keep abreast of all federal, state, and Rowan University regulations, laws, and policies as they presently exist and as they change or are modified.

Requirements:

This successful candidate will work on a mobile health unit in Cumberland County, New Jersey. The hours will be 37.5 per week (full-time). This role is a 6-month temporary position with renewal possible. Must have a valid Driver’s License. 

Qualifications:

Strong working knowledge of Microsoft Office. Licensed Clinical Social Worker (LCSW) with a minimum of five years of professional experience; prior supervisory experience is preferred.   Working experience on grants serving populations in need. Ability to manage multiple projects and coordinate the efforts of a team to accomplish objectives.  Effective oral and written communication skills, as well as the ability to function in a team environment, are essential.

SALARY INFORMATION 
  • In compliance with The State of New Jersey's Pay Transparency Act, the annual base salary range for this position is $75,000-$85,000, with placement within the range determined by candidate's qualifications, professional experience, internal equity, and other factors.
