Job Summary:

In this position, the social worker will work directly with patients who may be victims of community violence and their families who present to the hospital, utilizing evidence based trauma informed care.  The hospital-based SNUG (Should Never Use Guns) Social Worker will work closely with the Syracuse Community SNUG Site Program and the Division of Criminal Justice Services to improve access to victim services.  SNUG follows the Cure Violence Model of violence prevention dedicated to reducing gun violence in high risk neighborhoods and on behavioral changes in the target population to prevent victimization and perpetuation of gun violence.  Social Workers provide comprehensive psycho-social assessment, management and support to at risk patients and families. In addition, the social worker addresses social determinants of health and works closely with the multidisciplinary team.  The candidate shall gain knowledge of social determinants, population health and drivers of utilization. Duties will include consultation through assessment, counseling, intervention, planning and referrals to meet the psycho-social needs of patients and families.  Candidate must possess communication skills, have knowledge of available community resources, document all interventions; participate in departmental meetings/in-services, education, and quality improvement activities; and perform other relevant duties as assigned to include coverage for other social workers and back-up responsibilities.

Minimum Qualifications:  MSW from a Social Work program accredited by the Council on Social Work (CSWE); M.S.W., L.M.S.W., L.C.S.W. or eligible, 3 years relevant post-master's experience (preferably in a hospital and/or healthcare environment) required. New M.S.W. graduates who are L.M.S.W. eligible will be considered for appointment to entry level (SL2) position at a commensurate salary after obtaining a Limited Permit. Incumbent must obtain L.M.S.W. within 12 months of receipt of limited permit.

Work Days:

Openings for Days/Evening/Nights and potential every other weekend coverage. Occasional on-call/holiday coverage on rotation based on operational need.
