Job Summary:

In this position, the social worker provides comprehensive psycho-social assessment, management and support to at risk patients and families on the neuroscience units at Upstate Medical University Hospital. Knowledge of social determinants, population health and drivers of utilization. Duties will include consultation through assessment, counselling, intervention, planning and referrals to meet the psycho-social needs of patients and families. Candidate shall learn to initiate and implement advance directives. In addition, the social worker will actively manager patients through collaboration, coordination, monitoring, evaluating options and identifying required services to meet individual’s health needs. The social worker rounds on their assigned patients with the multidisciplinary team. Additionally, the social worker will participate in quality meetings on their assigned unit. Candidate must possess communication skills, have knowledge of available community resources, document all interventions; participate in departmental meetings/in-services, education, and quality improvement activities; and perform other relevant duties as assigned to include coverage for other social workers and back-up responsibilities.

Minimum Qualifications:

M.S.W from a Social Work program accredited by the Council on Social Work (CSWE); M.S.W., L.M.S.W., L.C.S.W. or eligible, 3 years relevant post-master's experience (preferably in a hospital and/or healthcare environment) required. New M.S.W. graduates who are L.M.S.W. eligible will be considered for appointment to entry level (SL2) position at a commensurate salary after obtaining a Limited Permit. Incumbent must obtain L.M.S.W. within 12 months of receipt of limited permit.

Work Days:

Monday-Friday, 8:30a-5:00p with occasional on call/holiday coverage on a rotating basis to meet operational needs.
