Job Summary:
The social worker provides assessment, intervention and referral utilizing the Health Steps for Young Children (Health Steps) program. Applicant should have experience working with individuals from a multigenerational heritage of poverty and understand the multitude of Adverse Childhood Experiences (ACEs) leading to social and mental health challenges. Upstate Pediatrics and Adolescent Center (UPAC) serves patients/families with very limited English language proficiency, new refugees, and those with limited health literacy. Applicant should possess knowledge of community resources, E.G. Arise, Syracuse Refugee Center, Salvation Army, Syracuse Housing Authority, Huntington Family Center, Early Intervention, Onondaga County DSS, behavioral health agencies, etc. This position will be located at UPAC and the applicant will work closely with the interdisciplinary team. Applicant must possess communication skills, document all interventions in the electronic medical record, participate in departmental meetings, education, quality improvement activities and perform other relevant duties to meet operational needs of the department.
Minimum Qualifications:
M.S.W from a Social Work program accredited by the Council on Social Work (CSWE); M.S.W., L.M.S.W., L.C.S.W. or eligible, 3 years relevant post-master's experience (preferably in a hospital and/or healthcare environment) required. New M.S.W. graduates who are L.M.S.W. eligible will be considered for appointment to entry level (SL2) position at a commensurate salary after obtaining a Limited Permit. Incumbent must obtain L.M.S.W. within 12 months of receipt of limited permit.

Monday-Friday 8:30-5 Occasional on-call/holiday coverage on rotation. Based on operational needs.
