Housecalls for the Homeless is a street medicine program of Upstate Medical University that delivers medical care for men and woman experiencing homelessness in Onondaga County.
Join a dedicated team of medical providers, students, and staff to reach our local community of the unhoused. Our teams collaborate with various organizations engaged in helping those experiencing homelessness, to supply clinical support. We are seeking a candidate that will continue our efforts of outreach by scheduling/registering patients, working with case managers to connect patients to resources for healthcare, and ensuring the appropriate workflows are abided by our teams.
Registration and scheduling of patients in providers schedules as needed
Communicating with case managers of the organizations we collaborate with to achieve the highest level of care for our patients. This includes coordination for shelter visit dates, receiving calls for patients wishing to be seen, medication refill requests, and follow-up appointment dates.
Monitoring and creating medical teams for our shelter visits
Overseeing the workflow and needs of the various outreach locations, resolving issues as necessary.
Taking stock of inventory (medical supplies and medications) for the various outreach locations through item sign-outs by our medical teams, refilling/reordering as necessary
Visiting sites 1-2 times/month to ensure workflow is being abided by and our teams are fulfilling their expectations.
