The Social Worker's primary responsibility is to function as a Patient Centered Medical Home (PCMH) social worker at University Internists and the Upstate Family and Preventive Medicine offices at both campuses. The process may include behavioral health conditions, high cost/high utilization, poorly controlled or complex conditions, social determinate of health, and referrals to outside organizations. Through the use of a wide range of interventions and treatment modalities, the Social Worker contributes to the achievement of optimum health, satisfaction and cost-effective care by addressing the behavioral, social and environmental issues which may impede care.
