The RN Navigator is a Registered Nurse whose clinical nursing experience guides patients and their families to informed decision making. The RN Navigator will work with the Thoracic Oncology population. Assumes responsibilities and accountability for planning, guiding and evaluating the delivery of patient care. Monitors patient's progress throughout treatment. Intervenes as needed to ensure compliance and optimum care. Collaborates within a multidisciplinary team to facilitate timely, appropriate screening, diagnosis, therapeutic intervention, treatment and supportive care across the health care continuum. The RN Navigator assumes responsibility for the coordination of all resources necessary to provide care. Utilizes advanced expertise to identify, implement and improve processes in order to provide an integrative model of care to the patients, while assuring the highest level of patient satisfaction. The RN Navigator provides patient education for each patient regarding their disease process and treatment throughout the health care continuum. Demonstrates and promotes professional development. Maintains a professional, safe and therapeutic environment for rendering care. |

Clinical Documentation Improvement Specialist

The Clinical Documentation Improvement (CDI) Specialist primary responsibility is to ensure the inpatient medical record accurately supports severity of illness and risk of mortality, as well as reflects the extent of care being provided and the resources being utilized for patients within the University Hospital system, utilizing comprehensive and broad proficiency in complex clinical and medical concepts. This is achieved by both thorough comprehensive concurrent and retrospective medical record review for accuracy, specificity, and validation of clinical diagnoses through application of clinical, documentation, and ICD-10 coding conventions, as well as Centers for Medicare and Medicaid Services (CMS) rules and regulations. When inconsistencies, incomplete documentation, or opportunity for medical record clarification or optimization are identified, the CDI Specialist will collaborate with the medical team to clarify necessary items through a dedicated and compliant written or verbal query process. The CDI Specialist is also responsible for collaboration with the inpatient coding team to ensure the assigned final code set is reconciled pre-bill, when necessary, to resolve necessary discrepancies, as well as provider education related to documentation concepts, both written and verbal, achieved through both on-site and virtual education platforms, including in-person rounds with the interdisciplinary care team and attend various departmental and committee meetings. |

The Nursing Quality Coordinator (NQC) will report to the Nursing Quality and Safe Patient Handling Program Manager. This role will oversee all nurse-sensitive indicators to include all hospital patient falls-witnessed, unwitnessed, & falls with injuries, Catheter-Associated Urinary Tract Infections (CAUTI), Central Line Associated Blood Infections (CLABSI), Clostridium Difficile Infections (CDIFF), and Hospital Acquired Pressure Injuries (HAPIs). The NQC, will review high-incidence units of the identified NSI and provide nursing education, complete root cause analysis, data analysis, and patient care outcomes. The role is to be a leader in the implementation of evidence-based practices impacting nurse-sensitive quality indicators. Provides education, training, clinical support, and collaborating with nursing leaders and bedside staff in establishing, identifying, and developing standards for evidence-based nursing practices impacting quality outcomes. Partners with nurse leadership, nursing staff, providers, families, and caregivers to review case studies, identify process improvements and implement evidence-based clinical interventions in a safe, effective, patient-centered manner. Collects and analyzes comprehensive data related to nurse-sensitive quality adverse events, and assesses data for educational gaps and opportunities for improvement. Must have the ability to analyze and interpret data which is essential to facilitate, design, and develop process improvement initiatives based on findings. The role will focus on care that is safe, timely, effective, efficient, equitable, and patient-centered. |
