In this position, the social worker is assigned to provide coverage in the Inpatient Adult Psychiatric Unit at the Upstate University Hospital downtown campus. The unit serves adults aged 18 years and older who require short-term, acute inpatient mental health care.
Social Worker- Multi Visit Patient Program	Syracuse
The Social Worker's primary responsibility is to function as a Patient Centered Medical Home (PCMH) social worker at University Internists and the Upstate Family and Preventive Medicine offices at both campuses. This social worker works with individual patients to plan, manage and coordinate care based on need. Patients are identified that may benefit from care management services. The process may include behavioral health conditions, high cost/high utilization, poorly controlled or complex conditions, social determinate of health, and referrals to outside organizations. Through the use of a wide range of interventions and treatment modalities, the Social Worker contributes to the achievement of optimum health, satisfaction and cost-effective care by addressing the behavioral, social and environmental issues which may impede care.
