SHC CASE MANAGER 
Student Service Professional IV
Student Health Center 

Salary Range: $6,320 - $9,014/Monthly 

Work Schedule: Full-Time, Monday – Thursday, 8:30 a.m. to 6:00 p.m., Every other Friday 8:30 a.m. to 5:00 p.m.; emergency hire, ends on or before 6 months from date of hire; exempt classification.

Essential Functions: Under the general supervision of the Director of Counseling and Psychological Services (CAPS), the Student Health Center (SHC) Case Manager is responsible for performing complex, professional student services. This role involves reviewing challenging individual, group, and organizational cases, developing appropriate strategies, and implementing effective solutions. The SHC Case Manager provides direct support to students experiencing varying levels of psychological and medical needs, facilitating referrals to external resources when necessary. Working closely with SHC staff, mental health counselors and medical providers who have identified clients/patients in need of case management services, including connecting the student with mental health or medical services, and in coordinating their transition to these services. This position requires high-level of confidentiality, analytic and interpersonal skills for providing direct services to students/patients and for cultivating effective relationships with nearby off-campus medical and mental health providers/resources. The SHC Case Manager will be working collaboratively with the Dean of Students Office as well to ensure appropriate referral to any required-on campus resources and supports. The SHC Case Manager establishes and maintains a comprehensive list of outside health-related resources for students, documents referrals and tracks their effectiveness, provides pertinent student/patient education, and strives continuously to ensure the continuity of quality health care.

Required Qualifications & Experience: Equivalent to graduation from an accredited four-year college or university in a related field, including or supplemented by upper division or graduate course work in counseling techniques, interviewing, and conflict resolution. At least four (4) years of progressively responsible professional student services work experience which includes experience in advising students individually and in groups, and in analysis and resolution of complex student services problems. A master’s degree in counseling, Clinical Psychology, or Social Work from an accredited program or a job-related field may be substituted for one year of professional experience. A doctorate degree and the appropriate internship or clinical training in counseling, guidance from an accredited program or a job-related field may be substituted for two years of the required professional experience for positions with a major responsibility for professional, personal, or career counseling. The incumbent must have a demonstrated interest and/or ability to work in a multicultural/multiethnic environment. A background check (including a criminal records check) must be completed satisfactorily before any candidate can be offered a position with the California State University. Failure to satisfactorily complete the background check may affect the application status of applicants or continued employment of current California State University employees who may apply for the position.

Desired Qualifications: Master’s degree in social work or another related field. Experience in college health or ambulatory care, medical clinic or social service provider. Experience working in patient coordination of care and/or patient referral services. Ability to triage students/patients to determine the level of service needed. Experience with supervising bachelor’s-level students and/or other employees. Excellent customer service/customer care skills. Experience conducting outreach and presenting. Knowledge of HIPAA. Bilingual and biliterate in Spanish preferred.
