PLEASE NOTE: The starting salary placement depends on qualifications and experience and is anticipated to be in the range of $4,598.00 per month to $6,960.00 per month.

About the Position

Reporting to the Director of Counseling Services, the Case Management Coordinator serves as a primary resource for students of concern on campus. The position provides case management services to students with complex mental and physical health problems, learning and social challenges, difficulty navigating campus systems, and safety concerns who are reported to the Student Care Team. Case management services include but are not limited to providing assessment, intervention, advocacy, resources and referrals, and follow-up services. The position coordinates student services and provides referrals to the appropriate resources on campus such as Student Health and Counseling Services, Accessibility Services, Student Conduct, Rights and Responsibilities, Office of the Registrar, Financial Aid, and off-campus community agencies. The Case Management Coordinator works with Student Wellbeing Services to respond to emergency assistance requests and disburse emergency funding. The position also works with University Housing and Residence Life to place students in on-campus Emergency Housing, and coordinates case management and transition planning for after their short-term stay.
In addition to supporting students, the Case Management Coordinator is also available to consult with faculty, staff, parents, and others who are concerned about a student's behavior. The position works closely with the Student Care Team to assign, follow up on, and track referrals.
Responsibilities
Case Management & Student Advocacy Services:
- Provide case management of students with complex mental and physical health problems, academic and social challenges, difficulty navigating campus systems, and safety concerns.
- Facilitate response and assistance for students in personal crisis, family crisis or other emergencies.
- Serve as a member of the Student Care Team to receive referrals, follow up with students, and document actions taken. May also assign cases, coordinate efforts, and track progress by team members.
- Act and advocate for students as they navigates support services. Work with relevant parties to ensure the student’s needs are understood.
- Establish comprehensive plans of care and support utilizing appropriate on campus and community services. Re-evaluate and adjust care plan over time; partner with the student and collaborate with other relevant parties.
- Facilitate and track referrals.
- Coordinate the efforts of various providers, and facilitate communication among providers to ensure the student receives quality service.
- Work with students requesting emergency housing to assess appropriateness, coordinate placement, and develop transition to longer term placements within the required timeframe.
- Educate and advise the student in the development of self-advocacy skills.
- Offer education on financial literacy, advocacy skills, and relapse prevention as practicable based on student needs.
- Seek culturally competent services and service providers to meet the unique needs of each student.
- Serve as a consultant on supporting students of concern with faculty, staff, and administration.
- Respond to parent and community reports about students.
Tracking and Referrals:
- Track and follow up with students of concern who do not attend appointments, or who other departments are having difficulty reaching.
- Maintain contact with clients waiting for services.
- Coordinate communication, discharge planning, and referrals for students after hospitalization.
- Maintain case documentation in accordance with all applicable policies, laws and instructions.
- Provide oversight to the Student Care Team reporting system. Enter notes into the database to track action taken on specific cases. May also run reports to provide analysis of campus trends to the Student Care Team.
- Perform ongoing research of community services, and update university databases and referral lists.
- Initiate and develop on-going liaison relationships with community health providers, service hospitals, and other resources.
- Coordinate referrals between SHCS, campus and community services.
Training Development and Outreach:
- Participate in student outreach events to promote available resources. Engage in outreach with students, student organizations, residence halls and campus departments to promote awareness and understanding of the Student Care Team.
- Serve on selected department and university committees.
- Update website and produce promotional materials to educate campus about Student Care Team and other student support services.
Minimum Qualifications
- A Bachelor's degree and/or equivalent training and administrative work experience involving study, analysis, and/or evaluation leading to the development or improvement of administrative policies, procedures, practices, or programs.
- Requires general knowledge and skills in the applicable administrative and/or program field with a foundational knowledge of public administration principles, practices, and methods.

- Working knowledge of standard software applications: word processing, spreadsheet and database management, Google and Microsoft suites, and video conferencing.
- Demonstrated ability to work under pressure, and to complete projects successfully both independently and as part of a group.
- Knowledge of community services are resources related to housing, health care, and basic needs.
- Ability to occasionally work evenings and on weekends when required to manage crises or participate in pre-scheduled outreach events.
Preferred Skills and Knowledge
- A Master’s degree in social work, counseling, or related field preferred.
- Three (3) or more years of experience in adult case management services.
- Experience working on a multidisciplinary team.
- Experience presenting to diverse groups of students, faculty and staff.
- Familiarity with CSU campus processes and student support resources.
- Experience compiling aggregate data and writing reports.
