Overview: Reporting to the MDT Manager, the Access to Care Coordinator is responsible for overseeing the goals and objectives of the access to care program to include coordination of 24/7 Hotline, intake coordination, and general flow of access to care inclusive of Intake and Assessment scheduling and counseling referrals. This position ensures the delivery of high quality, trauma-informed support and coordination for individuals accessing mental health and advocacy services.
Duties and Responsibilities (other duties as assigned): 1. Coordination: Provides support to staff, trainees and volunteer advocates by providing consultation and/or coordinating with appropriate supervisor as needed. Oversees and coordinates coverage of the 24/7 hotline. Monitors assigned volunteer productivity and compliance with program standards. Provides performance evaluations with volunteers and, when necessary, develops corrective action plans for those volunteers whose performance does not meet agency standards. Meets regularly with supervisor and/or Executive Team to ensure coordination and communication regarding ongoing issues and concerns. 2. Crisis Intervention: Oversees the provision of immediate crisis support and de-escalation for individuals in emotional distress. Provides crisis intervention, accompaniments and appropriate referrals on the 24/7 hotline and supervision coverage as needed. 3. Intake Screening & Scheduling: Supervises the intake screening process to assess client needs and program eligibility. Oversees the scheduling and review of intake assessments (IAs) across all StrengthUnited programs and referral of clients to the therapy program in the organization. Works collaboratively with the Clinical Program Manager and the agency therapists, case managers, trainees and advocates. Maintains a master schedule and oversees the direct assigning of Intakes and assessments (IAs) to the IA team of clinicians to ensure effective and timely responses to requests for services. Maintains and monitors the therapy waitlist and works collaboratively with other Program Managers to ensure appropriate assignment based on client needs. 4. Resource & Referral Services: Guides the Access to Care team in researching and distributing up-to-date community resources. Maintains current knowledge of local services to provide effective client linkage and support. 5. Training and Mentoring: provides initial and ongoing training, support and mentoring to incoming volunteers regarding the hotline, accompaniments, and interview and assessment process. Reviews policies and procedures. 6. Onsite Partnership Liaison: While upholding confidentiality, communicates with onsite partners to advocate for clients; assists onsite partners in addressing clients’ needs; facilitates communication between onsite partners and clients; obtains police reports to assist in filing applications with the Victim Assistance Program; provides onsite partners with advocacy resources and materials; acts as a liaison between client and law enforcement; civil-legal services, victims assistance and medical staff professionals; coordinates with Family Justice Center (FJC) staff on a daily basis to maintain order, rapport, and respect. Maintains close contact with each client’s authorized collateral resources and supports. Participates in scheduled staff and multidisciplinary team meetings, presenting relevant information on all cases, changes, and onsite partner referrals. Completes follow-up on participants following the completion of services. 7. Data Collection and Documentation: In partnership with the specialist ensures all data is accurately completed and submitted in a timely fashion; documents all onsite and offsite referrals, and outcomes; gathers and enters statistical data resulting from advocate contacts into software system; audits software system on a monthly basis to ensure data align with applicant tracking forms(ATF). 8. Stewardship: Performs duties in a highly professional manner, advancing the organization’s mission and reputation in the community, upholding the organization’s values, core principles and policies. Responds to all internal and external requests in a timely fashion with high standards of oral and written communication. Advocates for the organization’s mission through participation in workgroups, task forces, and other related meetings. Collaborates with onsite multi-disciplinary partners to deliver high performing programming while meeting confidentiality standards. Other duties as assigned.
Qualifications: Bilingual Spanish speaking required. Bachelor’s or Master’s degree in Social Work, Psychology, MFT, or related clinical field required Important: This position requires clinical knowledge and experience but is NOT a clinical role. Must be a highly organized individual able to multi-task, track and document assignment of clients. Computer competency, including Word, Excel, Outlook and PowerPoint. Experience with a database management system.
Knowledge, Specialized Skills, and Abilities: Experience with crisis intervention, family systems, and biopsychosocial assessment highly preferred. Must possess excellent written and communication skills and
have a basic knowledge of body language and non-verbal communication. Understands and demonstrates active listening skills. Analyzes complex situations accurately and adopts effective courses of action; supervises staff, student trainees and volunteers individually and in groups on complex matters when needed; determines appropriate courses of action and proper techniques to utilize while engaged with individuals in interactions of a sensitive nature; and interpret and evaluate descriptions and explanations of problems. Must have the ability and specialized skills to organize, coordinate, and prioritize multiple tasks; work independently in a busy environment with frequent interruptions and changing priorities; maintain cooperative working relationships with others; be team oriented; coordinate/collaborate with professionals from multiple agencies while maintaining appropriate boundaries and client confidentiality. Must show a passion and sensitivity for trauma work. Must demonstrate an acceptance and respect for cultural diversity in all its forms, including ethnicity and sexual orientation, abilities, and religious backgrounds and an ability to work collaboratively in an ethnically linguistically, and culturally diverse environment.
Pay, Benefits, & Work Schedule
This position is employed through The University Corporation (TUC).
The anticipated hiring range: $26.92 - $28.00 per hour
Time Base: Full Time
