Reporting to the Director of Counseling Services, the Case Management Coordinator serves as a primary resource for students of concern on campus. The position provides case management services to students with complex mental and physical health problems, academic and social challenges, difficulty navigating campus systems, and safety concerns who are reported to the Student Care Team. Case management services include but are not limited to providing assessment, intervention, advocacy, resources and referrals, and follow-up services. The position coordinates student services and provides referrals to the appropriate resources on campus such as Student Health and Counseling Services, Accessibility Services, Student Conduct, Rights and Responsibilities, Office of the Registrar, Financial Aid, and off-campus community agencies. The Case Management Coordinator works with Student Wellbeing to respond to emergency assistance requests and disburse emergency funding. The position also works with University Housing and Residence Life to place students in on-campus Emergency Housing, and coordinates case management and transition planning to longer term placements. |
