Position Summary
The Department of Orthopedic Surgery is hiring a Clinical Care Coordinator II (Registered Nurse). The Clinical Care Coordinator II is part of a collaborative nursing team providing day-to-day supervision of clinical activities of assigned providers.
Responsibilities
Essential Duties
- Screen new patient phone calls, request medical records and imaging to be sent in advance for review; assist patient with scheduling appointment for consultation with provider.
- Oversees appointment bookings and ensures emergency situations are scheduled appropriately.
- Coordination of surgical date with attending and patient, reschedule surgeries as needed, prioritize surgeries in emergent cases.
- Provide pre-operative patient education and support; including explanation of diagnosis, upcoming testing and surgical procedure.
- Schedule/order pre-operative testing, review completed testing results and report any abnormality to provider.
- Prepare charts and imaging for upcoming surgery.
- Interface between providers, resident staff as a well as the nurses, therapists, and other support personnel who take part in caring for surgical patients.
- Coordinate and facilitate communication between adult hospital, children's hospital and clinic facilities.
- Acts as a liaison in recovery room setting on surgical days in combination with recovery staff and provider.
- Facilitate the patient's transition within and between healthcare settings, e.g. admitting, transferring, discharging and home care including visiting nurse referrals.
- Sustain knowledge of provider's protocols and post-surgical precautions; implement protocols and update all staff as needed.
- Review post-operative discharge instructions with patients including regarding wound care and pain management.
- Round independently and/or with provider on all orthopedic inpatients.
- Triage daily patient phone calls throughout the day; instructs patient and family regarding treatment instruction, documents action taken in the Electronic Medical Record (CROWN).
- Address patient concerns and issues; assess, prioritize and subsequently resolve each matter. Report any patient related issues to provider.
- Fill out necessary forms, i.e. disability forms, workman's comp, handicap parking as needed; Assist with patient correspondence as needed, i.e. letters of medical necessity, school notes, insurance inquiries, etc.
- Assist provider during patient hours with examinations, prescriptions and direct patient care, remove/apply sutures and dressings.
- Send patient telephones prescription requests to pharmacy and documents action in the Electronic Medical Record (EPIC).
- Chart preparation according to protocol and orders appropriate x-rays for office hours using the Electronic Medical Record (EPIC).
- Contact all patients and fax/email requisitions for outside radiology orders as needed.
- Review schedules prior to office hours and orders x-rays for add on patients using the Electronic Medical Record (EPIC).
- Greet and escort patients and/or guardians to the examination rooms.
- Ensure patients have completed all paperwork; obtain vitals and enter data into the Electronic Medical Record (EPIC); complies with Meaningful Use Measures.
- Communicates as necessary and appropriate with primary care or local physician any care plan being initiated for patients.
- Acts as a scribe to provider as needed and completes encounter forms.
- Disposes of contaminated items according to OSHA guidelines.
- Assist patients in meeting health needs by identifying potential or existing problems, exploring available alternatives and providing support to the patient and family members to make an informed decision regarding health care.
- Formulate patient care protocols and critical pathways.
- Assist with development; participate in design and implementation of clinical research trials.
- Participate in accordance with hospital policies.
- Maintains patient education and instruction materials for distributing to patients.
- Maintains examination/consultation/treatment room supply inventory and tracks and orders supplies as needed.
- Maintains medication/injection inventory and follow-up.
- Participates in professional development activities and maintains professional affiliations.
- Maintains and attends BLS, CPR, OSHA, and CLIA training programs and certifications as needed.
- Participates in professional development activities and maintains professional affiliations.
- Attends external and internal conferences; Grand Rounds, Indications, etc.
- Maintains patient confidentiality; complies with HIPAA and compliance guidelines established by the hospital.
- Attends all regular clinical team and departmental meetings
- Works as part of the orthopedic clinical team and offers cross-coverage to other staff members or sites as needed.
- Maintains and utilizes monthly team budget to enhance the teams overall moral, environment, resources and/or tools; secures and submits proper reimbursement forms and receipts.
- Performs scribe duties as needed.
- Other duties as assigned within scope of practice, experience, and education.
People
- Promotes staff professionalism and performance with coaching, training and feedback. Mentors others in individual and team accountability, modeling behavior and demonstrating best practices/techniques. Demonstrates self-development and keeps current on a variety of clinical management and health care topics.
- Assists with interviewing, hiring and onboarding of new clinical team staff, including nurses, medical assistants, front desk staff and administrative personnel.
- Assists with the physician recruitment and termination process; sets up/terminating new physician office including but not limited to creating clinical team protocols, doctor specific office and medical supplies, lab coats, NYS Official Prescription pads, physical therapy prescriptions, research and other forms and paperwork for office hour sessions.
- Coordinates monthly staff schedules to ensure all patient hours are covered, all other duties are completed and overtime is avoided whenever possible. Tracks vacations and sick days, and ensures all posts are filled during employee absences.
- Provides support to department Human Resources on staff recognition and other HR related programs.
Additional duties
- Oversees and documents the implementation and compliance with related institutional policies, procedures, programs, protocols, and systems to provide an efficient and accurate patient encounter which improves the practice effectiveness and the quality of patient outcomes. Ensures compliance with all regulatory agencies, departmental and organization policies and procedures for all staff.
- Keeps current on all organizational, regulatory compliance and patient privacy trainings policies (e.g. infection control, public safety, EH&S, HIPAA, OSHA, etc.) and successfully complete all required trainings.
- Ensures compliance for a safe and secure work environment at all sites by following all OSHA, EH&S and public safety policies and guidelines.
- Maintain patient privacy and confidentiality according to HIPAA requirements at all times.
Minimum Qualifications
- Bachelor of Science degree in Nursing or equivalent of education and experience
- A minimum of 4 years of related experience.
- Current and unrestricted New York State RN license.
- Must have excellent clinical skills in a high-volume, high-pressure environment.
- Candidate must demonstrate strong customer service orientation and the ability to deliver consistent exceptional service while demonstrating tact, respect and sensitivity.
- Candidate must be organized and be able to manage a demanding workload and moderately complex cases in an accurate and timely.
- Demonstrated advanced skills in problem assessment, and resolution and collaborative problem solving in complex, and interdisciplinary settings including advanced proficiency in nursing competencies.
- Ability to communicate effectively in both oral and written form. This position requires the ability to interact positively, constructively and effectively with professional staff, providers and organizational stakeholders.
- Ability to lead and mentor team through new and changing situations. Demonstrate a professional and compassionate manner while conveying a positive image of the practice.
- Ability to work independently and follow-through and handle multiple tasks simultaneously.

- Duties performed routinely require exposure to blood, body fluid, and tissue. Incumbent is expected to perform/assist in emergency care and/or first aid as needed.
- This role may see patients at multiple locations. Must be available to travel to other sites as needed and/or directed.
