The salary of the finalist selected for this role will be set based on a variety of factors, including but not limited to departmental budgets, qualifications, experience, education, licenses, specialty, and training. Position Summary
Candidate with a Licensed Master of Social Work (LMSW) is sought for a part-time position (8-10 hours a week) within the Department of Neurology’s Muscular Dystrophy Association Clinic.
Responsibilities
- Provide social work support for pediatric patients and adult patients who are cared for by the Muscular Dystrophy Association Clinic
- Complete forms required for referrals including homecare, in-home rehab services, respite, transportation, palliative care and hospice services, M11Q forms, durable equipment forms, and any other forms that can be completed within the social worker’s scope of practice.
- Compose letters of medical necessity.
- Provide case management, crisis intervention, and supportive counseling services to the individuals and families seen within the MDA clinic.
- Provide referrals and resources to patients and caregivers.
- Participates in staff meetings, in-services, administrative activities, and community outreach events.
Minimum Qualifications
- Must hold a master’s degree in social work, and 2 years of related experience.
- Must hold a New York State LMSW or LCSW license.
- Certificate from the New York State Education Department.
Preferred Qualifications
- Bilingual in English/Spanish strongly preferred.
Previous experience working with this population, or as a medical social worker.
