About the role
We are seeking a dedicated professional to coordinate service delivery for clients receiving CHSP and HACC PYP services, ensuring their assessed needs are met promptly and in line with relevant guidelines and Council policies. This role also involves assessing younger people with disabilities and supporting them in accessing appropriate services.
Key responsibilities include:
- Provide and update service data via the My Aged Care online provider portal and Carelink
- Develop and maintain effective referral, assessment and review mechanisms for prospective and existing service recipients.
- Undertake Occupational Health and Safety Assessments.
- Ensure clients and their carers are informed of rights and responsibilities, type and levels of care to be provided, the costs involved and grievance procedures.
- Provide and maintain up-to-date knowledge of developments in Assessment and Care Management Practices for the frail aged, people with dementia, people with disabilities and their carers.
- Ensure consistent, effective and efficient administrative systems are maintained. Ensure regular collection and reporting of statistical and other data.
About you
To be successful in this role, you will have experience and knowledge of issues relating to frail aged people, people with dementia, people with disabilities and their carers.
Essential Experience:
- Awareness and support of the philosophies of healthy ageing, promotion of independence, non-intrusive care and general disabilities.
- Extensive knowledge of governmental and community resources available for older people, people with dementia, people with disabilities and their carers.
- Comprehensive knowledge of holistic assessment and care management practices
- Program planning and development skills.
- Demonstrated experience in Assessment practice and preparation of Care Plans.
