Your role will sit within our Support division in the Ceredigion area helping facilitate a way for people to live a more fulfilling life. You will work with your team to truly make a difference in the lives of people we support.

Your day will consist of:

• Conducting needs assessments and person centred support plans
• Provide practical and emotional support to the individuals you support
• Provide support around housing challenges, mental health issues, substance misuse issues and other complex needs. 
• Assisting with sustaining and securing suitable accommodation
• Maintaining accurate and up-to-date records
• Developing your networking skills by working in partnership with external agencies
• Delivering workshops to the individuals we support.

Who we are looking for:

• Previous experience in a similar role
• A passion and commitment to supporting vulnerable individuals and promoting independence and wellbeing
• Organisation and time-management skills
• Ability to work independently as well as part of a team
• A non-judgmental and empathetic approach

What are the pay and benefits? 

• Part-time - 24 hours, permanent position
• Competitive salary of £24,531.00 FTE (£12.75 per hour)
• Work pattern: predominantly Monday – Friday office hours although flexibility is required to meet the needs of our customers.
• An enhanced DBS is required for this role which we will pay for
• 26 days (pro rata) holiday entitlement plus Bank holidays 
• Pension scheme

• 45p per mile
