About Your Role

Scope has an exciting and unique opportunity for a Service Transition Coordinator to work within our Employment Programs division.

In this role, you will coordinate the transition of new clients into Scope services, ensuring a smooth, well-supported onboarding experience that aligns with their individual goals and needs.

Key responsibilities include:

  • Acting as a central point of contact for clients, families and stakeholders during onboarding
  • Gathering and documenting client goals, preferences and support requirements in line with funding and compliance standards
  • Coordinating onboarding activities such as service matching, site visits, planning transitions and funding arrangements
  • Working collaboratively with internal teams and external stakeholders to ensure seamless service entry
  • Maintaining accurate records and ensuring documentation meets regulatory and organisational requirements
  • Providing clear communication regarding services, expectations and funding arrangements
  • Identifying opportunities to improve onboarding processes and client experiences
  • Following quality, safeguarding and risk management frameworks in all activities

What We Look For

  • Experience working with multiple stakeholders to deliver coordinated outcomes
  • Strong time management and prioritisation skills
  • Effective communication skills across a variety of formats
  • Experience in disability services, community services or a related sector (desirable) 
  • Experience in a not-for-profit environment (desirable)
