About Your Role
Scope has an exciting and unique opportunity for a Service Transition Coordinator to work within our Employment Programs division.
In this role, you will coordinate the transition of new clients into Scope services, ensuring a smooth, well-supported onboarding experience that aligns with their individual goals and needs.
Key responsibilities include:
- Acting as a central point of contact for clients, families and stakeholders during onboarding
- Gathering and documenting client goals, preferences and support requirements in line with funding and compliance standards
- Coordinating onboarding activities such as service matching, site visits, planning transitions and funding arrangements
- Working collaboratively with internal teams and external stakeholders to ensure seamless service entry
- Maintaining accurate records and ensuring documentation meets regulatory and organisational requirements
- Providing clear communication regarding services, expectations and funding arrangements
- Identifying opportunities to improve onboarding processes and client experiences
- Following quality, safeguarding and risk management frameworks in all activities

What We Look For
- Experience working with multiple stakeholders to deliver coordinated outcomes
- Strong time management and prioritisation skills
- Effective communication skills across a variety of formats
- Experience in disability services, community services or a related sector (desirable)
- Experience in a not-for-profit environment (desirable)
