The Role
The Social Worker supports clients and their families within the Transition Care program, ensuring their psychosocial wellbeing during transitions between home and residential care. They provide counselling, information, and guidance on care options, working closely with the multidisciplinary team to help clients achieve their goals. This role is key in ensuring person-centred, value-aligned support throughout the transition journey.
This position is offered as a 10-month fixed-term, part-time role, working 30.4 hours per fortnight. Hours are 8:30am to 4:45pm Thursday & Friday, based at Kinross Care Community – Beach House.
Key responsibilities:
- Conducting social work assessments, including psychosocial assessments, for assigned residents and clients in collaboration with the care team
- Planning and delivering social work interventions alongside residents, clients, and their families
- Coordinating care transitions, including counselling, advocacy, mediation, referrals, and support when moving clients to their home or a Residential Care facility
- Collaborating with residents, clients, and families to achieve positive outcomes, taking into account psychosocial factors
- Providing consultation and support to Nursing and Allied Health teams on the psychosocial needs of clients and residents
- Reviewing care goals and discharge plans to ensure relevance and continuous improvement
- Documenting client outcomes and sharing relevant updates with the broader care team
- Monitoring and evaluating the effectiveness of social work interventions to ensure they continue to meet individual needs
- Actively participating in regular multidisciplinary team meetings
- Communicating effectively with both internal and external stakeholders
- Ensuring compliance with Amana Living policies, procedures, and accreditation standards
- Attending required training and team meetings
- Contributing to the ongoing improvement of personal and professional work practices
About you
The suitable candidate will be a qualified Social Worker with experience in psychosocial support, care coordination, and client transition planning. The ideal candidate will be highly organised, able to manage multiple priorities, and work effectively both independently and as part of a multidisciplinary team, with a proactive attitude and the ability to drive positive change.
Essential Criteria:
- Evidence of COVID-19 and Current Flu vaccinations per Amana Living policies
- Ability to obtain a National Police Clearance (within 6-months validity)
- Successful completion of pre-employment physical and reference checks
- Bachelor of Social Work, current registration with Australian Association of Social Workers
- WA ‘C’ Class drivers license
- Demonstrated knowledge of current issues relating to social work
- Demonstrated ability to utilise a variety of social work interventions
- Previous experience within the Aged Care sector (Desirable)
