Part Time Role

 

The Opportunity

As a Social Worker in our hospital, you will be responsible for providing emotional support, counselling, practical assistance to patients and their families and ensure a smooth discharge. You will work closely with the healthcare team to ensure that patients receive the best possible care and support during their hospital stay.

 Your duties will include:

  • Provide emotional support, counselling, and practical assistance to patients and their families.
  • Work closely with healthcare professionals to identify patient needs and develop appropriate care plans.
  • Conduct assessments and coordinate appropriate community resources for patients who require ongoing support.
  • Develop and implement discharge plans for patients, ensuring a safe and smooth transition to home or a community setting.
  • Provide education and support to patients and families regarding hospital policies, procedures, and available resources.

 

Skills and Experience

  • A bachelor’s or master’s degree in social work
  • Relevant experience working as a Social Worker in a hospital setting, with experience in an acute ward setting.
  • Eligible to be an accredited member of AASW.
  • Demonstrated excellent interpersonal, communication, and problem-solving skills with consumers, their families and all health care professionals. 
  • Ability to work collaboratively within the multidisciplinary team including internal and external agencies.
  • Ability to work independently, flexibly and manage time effectively, prioritize competing demands.
  • Knowledge of the Aged Care sector will be highly regarded.

Requirements

  • Must provide a National Police Check conducted within the previous 12 months
