The Opportunity
As a Cancer Care Coordinator, you will be navigating all cancer streams but in particular breast care. You will be providing comprehensive support to patients diagnosed with breast cancer throughout their treatment journey. This role involves guiding patients through complex healthcare systems, scheduling appointments, facilitating referrals, and providing education about diagnoses, treatments, and available resources. The Cancer Care Coordinator acts as a central point of contact for patients, families, and multidisciplinary healthcare providers, ensuring seamless communication, efficient care delivery, and emotional support.
Your duties will include (but not limited to):
- Assist where appropriate, coordinate and schedule patient appointments and treatments.
- Serve as the primary point of contact for patients and families.
- Facilitate communication between patients and multidisciplinary healthcare teams.
- Provide education and resources about different cancer streams including breast cancer diagnoses and treatments.
- Advocate for patient needs and address barriers to care.
- Monitor patient progress and follow up on care plans.
- Assist with referrals to specialists and support services.
- Maintain accurate and confidential patient records.
- Support patients emotionally throughout their treatment journey.
- Collaborate with oncologists, nurses, and social workers.
Skills and Experience
- Current registration with AHPRA as a Registered Nurse
- Minimum of three (3) years’ experience in oncology nursing or relevant experience in advanced practice
- Post Graduate qualifications in Oncology or related field will be highly regarded
- Ability to work effectively in a multidisciplinary team environment and independently as required
- Highly effective communication and interpersonal skills
- Proven project management skills with sound organisational and change management skills
- Intermediate to advanced computer skills
Remuneration
On offer is an hourly salary of $55.17 + superannuation, exclusive of penalties or allowances. (NSW Clinical Nurse/Midwifery Specialist)
