About the Role 
  • The Rehabilitation Assessor (RA) acts as a clinical and professional resource person demonstrating broad knowledge and clinical expertise in rehabilitation programs (inpatient/outpatient)
  • You will complete patient pre-assessments in conjunction with the rehabilitation multidisciplinary team; to determine patient suitability and readiness for commencing rehabilitation.
  • You will also be responsible for facilitating community and acute patients into the appropriate rehabilitation stream whilst ensuring the patient and their carer are fully informed of the available rehabilitation services and processes. 
  • Also support rehabilitation bed coordination, patient flow and have a strong commitment to quality care, continuous improvement and excellent communication to all stakeholders (internal/external).
 
 
About You 
 
Essential:
  • AHPRA registration as a Registered Nurse, Endorsed Enrolled Nurse or Allied Health professional
  • Superior communication, negotiation, consultative and interpersonal skills, able to build strong business relationships at all levels  with private and public sector partners
  • Good understanding of the Private health funds, TAC, WorkCover etc.
  • Able to travel to other facilities
  • Strong teamwork and leadership skills, able to work autonomously whilst contributing to the team
  • An understanding of private health rehabilitation admission criteria
  • Biopsychosocial/rehabilitation clinical reasoning and assessment skills
  • Understanding of Functional Independence Measure (FIM)
  • Experience of complex case management and clinical decision making
  • A commitment to client centred interventions
  • Proficient computer processing skills
  • Proficient documentation skills
 
Desirable:
 
  • Successful completion of FIM education
  • Knowledge of private health sector funding models
  • At least 2 years post registration with a broad clinical knowledge base
