RESPONSIBILITIES:

I. Direct Patient Care Duties:
A. Assesses and charts observations of the patient’s condition at each visit.
B. Completes evaluation tasks, including reviewing medication, vital signs, and complete head to toe assessment.
C. Administers skilled nursing treatments and interventions including but not limited to at-home IV infusion, wound assessment, wound care, and education aimed at promoting and maintaining patient independence.
D. Revises plan of treatments and discharge plan in conjunction with registered nurse case manager, patient, family, physician, care delivery team, and third party payors including obtaining medical orders and approval of medical services.
E. Provides an environment, which promotes respect for the patients, their privacy, confidentiality and property.
F. Initiates appropriate preventive and rehabilitative strategies.
G. Consults with and educates the patient, family and other team members regarding disease process, self-care techniques and preventive strategies.
H. Interacts with patients, physicians, referral sources, and others in a manner conducive to continued positive relationships.
I. Assures the completion of all appropriate clinical records needed for compliance with state and federal legislation and the delivery of seamless service within established timeframes.
J. Maintains current knowledge of all available URMHC and community-based services.
K. Demonstrates flexibility and willingness to visit patients based on need and not geography.
L. Ensures problem resolution at the point of contact.
II. Participates in the attainment of the Agency operational and financial objectives.
A. Monitors results, progress, and takes corrective action on quality indicators, including the level of customer satisfaction with the RN function.
B. Understands the importance of the RN function and its impact on patients, other care providers, readmission, productivity, utilization, revenue and expense.
C. Pursues efforts to reduce or eliminate avoidable costs and errors.
D. Responds to findings of various Quality Management reviews/audits and ensures resolution of potential problem areas.
E. Participates in peer review and Quality Assessment and Performance Improvement as assigned.
F. Maintains productivity requirements.
G. Ensures that appropriate visit authorizations are in place and that visit documentation reflects the medical necessity for skilled nursing care to be provided in the home setting.
III. Contributes to an environment of professional growth, learning, trust and mutual respect for all employees.

B. Participates in education including orientation, in-service training programs and self-development, as needed.
C. Demonstrates commitment, professional growth and competency.
D. Responsible for the Key Performance Indicators (KPI’s) established by their manager.
E. Performs other duties as requested.
 

QUALIFICATIONS:

1. New York State Licensed RN.
2. One year of experience as a clinical nurse in a hospital or other direct patient care clinical setting required.
3. Strong critical thinking skills, the ability to follow standards/procedures, and make appropriate clinical judgments to provide safe and effective client care.
4. Ability to work independently with minimal supervision, adapt to change, and manage a variety of assignments while establishing priorities and ensuring the necessary duties are completed.
5. Strong interviewing, clinical assessment and communication skills.
6. Ability to exercise initiative and independent judgment.
7. Ability to build and maintain professional relationships
8. Current NYS driver’s license

Salary Range: $ 73,652.80 - $ 86,548.80
