Mater has an exciting opportunity for a Social Worker to join our Mater at Home team. We are searching for an exceptional and experienced Social Worker on a permanent, full-time basis.

The purpose of the role is to deliver a quality social work community service to the Mater at Home client population, as part of a multidisciplinary team.

As a member of the Mater at Home team you will deliver high quality community-based Social Work services to a diverse range of clients. This includes consumers of the NDIS and other government funding schemes (including Commonwealth Home Support Program, Post-Acute Care, Home Care Packages, and Hospital in the Home).

Salary range: $38.25 - $56.06 per hour plus 11.5% superannuation and access to not-for-profit salary packaging up to $15,900  + access to many staff benefits! 

What you’ll be doing

Key duties and responsibilities will include:

  • Delivering competent to complex level social work services in the community to adult residents in the Greater Brisbane Area (which includes psychosocial assessment, timely and appropriate social work interventions, referral and liaison with relevant services and advocacy to promote and address the needs of clients and their families).
  • Apply independent clinical judgement to a complex caseload under the professional guidance of a more senior clinician.
  • Work cooperatively and effectively within the multidisciplinary team.
  • Manage a workload utilising effective time management and prioritisation skills for clinical and non-clinical responsibilities and to meet determined targets for productivity, quality, safety, stewardship and client satisfaction.
  • Commit to patient safety and quality in the delivery of healthcare by actively participating in safety and quality improvement practices.
  • Provide a clinical and professional liaison role with the wider healthcare team involved with the patient/client, including external service providers.
  • Actively develop clinical and professional practice knowledge and expertise by engaging in ongoing professional development activities.

About you

To be successful in this role you will have:

  • A tertiary degree in Social Work with eligibility for membership of the Australian Association of Social Workers (AASW).
  • A current open driver’s licence, valid for use in Queensland.
  • A current Blue Card (Working with children and young people)
  • Current, or ability to obtain, NDIS Worker Screening Check (yellow card)
  • Experience working in community, aged care and / or disability environment is desirable but not necessary.
  • Interest or previous experience working in oncology and palliative care is desirable but not necessary.
