Clinical Nurse - Multicultural Health Coordination Apply now Job no: 502901 Work type: Permanent Location: South Brisbane Categories: Clinical Nursing/Midwifery About the role Mater Refugee Health Service is seeking a Clinical Nurse (CN) to join the Multicultural Health Coordination team working part time 0.4FTE on a permanent contract. The hours of work will be Monday – Friday between 08:30 – 17:00. We require applicants to be available for minimum 0.4FTE (2 days per week) and able to work in Brisbane, Logan and Ipswich. This is an exciting opportunity to join a high-performing team that includes specialist refugee and multicultural health nurses, doctors, social worker, administration staff and community engagement workers who are passionate about health access and equity for people of refugee and migrant backgrounds. Mater Multicultural Health Coordination Nurses provide nursing care, coordination, and health advocacy to individuals from refugee backgrounds in a variety of settings, including General Practices across Brisbane, Logan and Ipswich, the Mater Refugee Complex Care Clinic, community clinics and home environments. Salary range: $54.44 - $58.30 per hour plus 11.5% superannuation and access to not-for-profit salary packaging up to $15,900 + access to many staff benefits! What you’ll be doing Reporting to the Midwifery/Nursing Unit Manager your duties will include but not be limited to: Provides high quality patient care, where advanced clinical skills and a broad developing knowledge underpin care coordination, collaboration, and communication for excellence in patient outcomes Demonstrated high level of knowledge and awareness of the health needs of individuals from a culturally and linguistically diverse background Conducts systematic, holistic health assessments of patients of a culturally and linguistically diverse background using an outcome-based framework Implements evidence-based and culturally responsive nursing interventions for patients of a culturally and linguistically diverse background and initiates health promotion and health literacy education and appropriate referrals Plans care in collaboration with individual patients, groups of patients and multidisciplinary team members using assessment data Evaluates effectiveness of care in achieving predicted outcomes and reviews plans in accordance with evaluated data and initiates effective discharge planning processes Collaborates with other health professionals and community services in the delivery of equitable health care and identification of systemic barriers for appropriate escalation Establishes priorities for resolution of identified health needs in consultation with the patient or group of patients Participates in the development and implementation of innovative models of care that promote health equity, continuity, including review of existing models of care and attending practice visits to new community GP practices Acts as a patient advocate ensuring patients are informed and ‘informed’ consent is gained prior to treatment, investigation and management as appropriate and assists/negotiates sensitively with patients where there may be barriers to understanding About you You will be driven by a challenging yet rewarding environment where you can bring your energy and experience to this dynamic team. To be successful in this position, you will have: Current AHPRA nursing registration as a Registered Nurse (no restrictions) Class C driving license Minimum 3 years' clinical experience in at least one of the following specialist nursing fields: refugee health/ primary care/ rehabilitation/ disability/ paediatrics/ geriatrics/ chronic disease/ mental health Understanding of trauma-informed care and health equity and access issues for disadvantaged populations, with demonstrated experience in supporting clients to navigate these Advanced written and verbal communication skills including experience of working with peoples from diverse cultural backgrounds; engaging interpreters in a clinical setting, and the ability to liaise with a variety of different stakeholders both internally and externally at various levels A strong work ethic, positive attitude and willingness to role model and promote the Mater values Demonstrated ability to operate both in a team environment and autonomously using initiative, sound reasoning and strong advocacy, listening, negotiation and decision-making skills High level of attention to detail and advanced level skills in coordination, time management, digital literacy, problem-solving and ability to escalate matters appropriately Commitment to ensuring the levels of service is always delivered to patients Desirable: Be a person of Culturally and Linguistically Diverse background Post-graduate studies (completed or currently enrolled) relevant to the role Applications close on Sunday 27th April 2025.
