About the role

Mater Health is seeking an enthusiastic and experienced General Practice Liaison Officer (GPLO) to join the team. This position is offered on a maximum term, part-time (0.6 FTE) basis for 8 months. Based at Mater’s South Brisbane Campus, working within the Mater Hospital Brisbane Clinical Service as part of the Ambulatory & Outpatient Services leadership team. 

The General Practice Liaison Officer is a critical role in the interface of primary health care and hospital care, linking in as part of the state-wide Queensland Health GPLO network to facilitate collaborative care, clinical communication and transfer of care, and interprofessional education and engagement.

What you’ll be doing

The General Practice Liaison Officer advocates, coordinates, designs and evaluates initiatives that facilitate appropriate clinical pathways and transfer of care processes.  

Key responsibilities include: 

  • Facilitate integration of services at the interface between general practice and hospital care 
  • Develop collaborative model of care initiatives that improve access to and transfer of care, including clinical handover and discharge from hospital 
  • Identify and address service gaps, especially at the interface between primary and specialist outpatient care.  
  • Develop and deliver tools, resources, information and training to support GP referrals, shared care and clinical handover  
  • Support GPs as an escalation point and communication pathway for information and education, and feedback on current practices and proposed initiatives 
  • Contribute to a collaborative culture, demonstrating empathy and respect for patients, GPs, hospital staff and other stakeholders and supporting interprofessional education and engagement 
  • Contribute to strategic development activities, developing and participating in collaborative partnerships with local Primary Health Networks and surrounding Health and Hospital Services.  
  • Participate in the state-wide GPLO network, working groups and project initiatives, building capacity and capability across the network.  

About you

The successful candidate will have:  

  • Demonstrated knowledge and experience in a health or community setting, with a good understanding of the clinical demands in primary care. Knowledge and understanding of Queensland Health and public hospital and health services is highly desirable.  
  • Qualifications in a health and/or health management field. Staff with a clinical background are encouraged to apply.  
  • Qualifications and experience in project management desirable.  
  • Demonstrated excellence in understanding and incorporating needs of patients, family members, internal and external customers in all activities.  
  • Demonstrated excellence in building and maintaining relationships. Ability to support strategic direction, engaging in information seeking/networking and exploration of external benchmarks. 
  • Ability to organise, effectively plan and deliver on completion of tasks and project work in accordance with key performance criteria and reporting requirements.  
  • Ability to understand data and develop innovative responses to ensure continuous improvement.  
  • Ability to maximise and facilitate opportunities for positive change  
  • Strong capabilities in Microsoft Office Suite and ability to learn new systems.
