About the role
Mater at Home is looking for a passionate CHAMP at Home Care Assistant to join the team on a Maximum Term Part-time position working 15 hours per week until June 2026. This position will be based at our South Brisbane campus.
CHAMP: Continuity of care by Health professionals attending Alcohol and drug problems and meeting Mothers' needs for Positive family outcomes.
The CHAMP at Home Care Assistant provides program support for the CHAMP at Home Clinical Care Coordinators within the CHAMP at Home service. CHAMP at Home provides six months postnatal support for parenting women who have identified substance misuse with varying psychosocial needs. The service works alongside women in a community based, outreach model providing clinical care and coordination.
Salary range: $34.02 - $35.66 per hour plus 12% superannuation and access to not-for-profit salary packaging up to $15,900 + access to many staff benefits!
What you’ll be doing
You will have the following responsibilities:
- Work collaboratively with CHAMP at Home Clinical Care Coordinators, Mater midwives, Mater at Home clinicians and administrative staff to ensure effective and efficient service delivery for mothers referred to the CHAMP service.
- Assist CHAMP at Home Clinical Care Coordinators with administrative and operational program requirements.
- Complete progress notes, referrals and data entry in a timely and accurate manner.
- Ensure ongoing accurate data collection and assist with submission of quarterly and annual reporting requirements to funding body and government organisations.
- Participate in the development of practices, procedures and protocols within the CHAMP at Home model, as well as in the identification, development and implementation of quality and service improvement activities that enhance the delivery of CHAMP services and the Mater at Home program.
- Actively develop practice knowledge and expertise by engaging in ongoing professional development activities.
About you
You will demonstrate the following key capabilities, knowledge and experience:
- Certificate III in Allied Health Assistance or equivalent/similar certificates/qualifications
- Currently completing tertiary education in midwifery/nursing/social work - highly desirable
- Current QLD driving license
- Current Blue Card (Working with children and young people)
- Relevant experience in an AOD and/or mental health settings
- Experience working in support roles with antenatal and postnatal women
- Strong organisational skills, including ability to manage and coordinate a range of tasks and competing priorities
- The ability to work effectively both independently and within a multidisciplinary team
- Strong communication skills both written and verbal
- Demonstrate positive interpersonal skills between clients, team members and external stakeholders
