About the role

Mater at Home is seeking a highly motivated and experienced Clinical Nurse to join our team on a Maximum term 6-month Part-time basis, working 15 hours per week.

Mater at Home is introducing a 7-day Navigator role to support Hospital bed flow and be a point of contact for internal and external referral areas.  Mater at Home is looking for highly motivated clinical nurses (multiple positions) to provide the Navigator role on weekends, with the potential for some weekday shifts as required.

The Mater at Home Clinical Nurse is a dynamic and self-directed role providing advanced clinical competency to patients.

This is a fantastic opportunity for those interested in contributing to patient flow and early discharge planning while working closely with our multidisciplinary Mater at Home teams. The role is well-suited to experienced Registered Nurses with strong clinical decision-making skills, experience in coordination, and a patient-centred approach.

The successful applicant will work across Mater at Home portfolios as needed.

Salary range: $54.44 - $58.30 per hour plus 12% superannuation and access to not-for-profit salary packaging up to $15,900  + access to many staff benefits! 

What you’ll be doing

  • Support early discharge/transfer from the hospital to our service.
  • Receive nursing referrals from QAS, make clinical decisions based on our inclusion criteria on whether these patients are suitable for our service
  • Prioritise different patients' care needs and manage staffing resources appropriately
  • Coordinate care across acute, home, and community settings
  • Liaise closely with the multidisciplinary team
  • Facilitate timely and patient-centred transitions of care

About you

Essential

  • Current AHPRA nursing registration as a Registered Nurse (no restrictions)
  • Current open driver’s license
  • At least 3 years’ experience as a Registered Nurse
  • Previous experience as a Clinical Nurse Navigator or hospital flow is desirable
  • Advanced clinical expertise in the Hospital in the Home setting
  • Excellent written and verbal communication skills and the ability to liaise with a variety of different stakeholders both internally and externally at various levels
  • Demonstrated ability to operate in a team environment and autonomously using initiative, sound reasoning and strong listening, negotiation, and decision-making skills
  • Strong attention to detail and organisational skills
  • Commitment to ensuring a responsive and streamlined HITH service is always delivered to patients.
  • Flexibility to work weekends and ad hoc weekdays
