About the role
 
Mater Private Hospital Brisbane has an exciting opportunity for a suitably skilled and enthusiastic Registered Nurse to relieve in the role of Discharge Coordinator, providing ad hoc leave cover for the team. 
 
Salary range: $54.44 - $72.58 per hour (RN2 - RN3) plus 12% superannuation and access to not-for-profit salary packaging up to $15,900  + access to many staff benefits!
 
What you’ll be doing
 
The Discharge Coordinator plays a crucial role by managing and coordinating the discharge process for patients. This role involves collaborating with healthcare professionals, patients, and their families to ensure a smooth transition from the hospital to post-acute care settings or home.
 
Responsibilities of the role include:
• Driving timely discharge planning for patients requiring expert community service knowledge and management to facilitate their discharge when community supports or discharge to Aged Care Facilities are required
• Providing education, upskilling and understanding of the discharge process to clinicians, ensuring the understanding of the financial and other impacts of delayed discharge to the patient and the organisation 
• Leading the difficult conversations around Informed Financial Consents, when required, for non-acute patients, ensuring patients and staff understanding
• Assisting in the development implementation and review of policies associated with the discharge process and services  
• Driving the delivery of innovative clinical practice models and therapeutic techniques
• Maintaining a working knowledge of Health Funds and their impacts on timely discharge 
• Ensuring VMOs and clinical teams are aware of the impacts of clear communication and accurate documentation around the patient’s acute status when managing patient Length of Stay and timely discharge
• Leading, participating, and collaborating with other key stakeholders in the design and implementation of continuous improvement activities and models of care for enhanced service provision
• Developing specialised education for complex issues around discharge planning impacting on patients and carers 
• Ensuring strong communication networks are maintained across all levels of the multidisciplinary teams within Mater and with community organisations
• Providing education, training, and support to the nursing teams.
• Participating in team huddles and meetings, as appropriate, to enhance patient care and staff information-sharing
• Linking community organisations, Aged Care Facilities, and the nursing teams, in ensuring patients receive timely and appropriate discharge.
• Identifying and actively communicating future issues and directions for the service. 
• Appropriately escalating to the executive team, any issues impacting on clinical practice and timely discharge that are unable to be solved utilising usual processes
• Leading and contributing to care coordination initiatives for the service. 
 
About you
The successful candidate will need to demonstrate advanced clinical expertise and knowledge of procedures and a basic understanding of health fund impacts on Length of stay, support the Nurse Manager in implementing strategies to ensure that quality and risk management are integrated into service provision and maintain current knowledge and skills to ensure continued professional growth.
