About the Role:
As a Care Assessment Coordinator, you will play a vital role in coordinating the AN-ACC (Australian National Aged Care Classification) process, leading pre-admission assessments, and ensuring optimal funding outcomes. You’ll work closely with our Residential Manager and Funding Consultants to support residents' care journeys and uphold the highest standards of aged care.

About You:
To be considered, you will have:
- A qualification as a Registered Nurse, along with experience in aged care and knowledge of the AN-ACC
- Strong interpersonal and communication skills
- Proficiency in MS Office and clinical documentation systems
- Ability to lead, train, and support staff
- Excellent time management and problem-solving skills
- Experience with ACFI and quality improvement tools - desirable
