As a Social Club Coordinator with BaptistCare you will be responsible for coordinating the delivery of high-quality care and services that provide our social club customers the opportunity to engage in meaningful activities and interactions with both staff and other customers in a safe and welcoming environment. Located at our social club in Ashfield, you’ll develop genuine connections with our customers and their loved ones to develop programs that optimise their health and wellbeing encourage engagement and support their independence.

A typical day will include (but not limited to):

  • Developing individualised care plans and activities that reflect customer’s needs, goals and preferences, promote independence and provide an excellent customer experience
  • Reviewing and updating care plans to meet customer changing needs and mandated requirements and incorporating innovative approaches to activities and service delivery
  • Supporting your direct reports through well planned and informative team meetings and regular 1:1 supervision
  • Actioning referrals within reasonable timeframes. This includes initial referrals for service delivery and referrals required during the customer’s episode of care
  • Delivering a timely, professional response to customer feedback and complaints and actively assisting with addressing and resolving these matters

What do we need from you?

You will need to be able to promote a team culture in which all staff and customers feel valued and respected, have excellent verbal communication skills and have an empathetic, patient and caring nature.

This role does require you to have:

  • Bachelor’s Degree in related field or Diploma of Diversional Therapy or Leisure and Health or Diploma of Community Services (Case Management), or equivalent qualification, knowledge and experience
  • Experience in facilitating social support group programs and activities within an aged care or similar environment
  • Experience supporting and leading a team through positive leadership and effective workforce planning and development
  • First Aid and CPR certificate or willingness to obtain
  • Demonstrated experience in operating within funding constraints and achieving service delivery objectives within government program guidelines
  • A valid NSW driver's licence
  • Intermediate level of computer skills
  • Excellent time management and organisational skills
  • Ability to work independently and as part of a team
