About the opportunity

An exciting opportunity exists for a Practice Improvement Specialist to use their skills and experience to implement, coach and guide the adoption of Uniting’s Senior Services models of care to improve service delivery outcomes. 

Required to travel throughout the Sydney Central area

Development/leadership opportunities

Supportive multi-disciplinary team  

Reporting directly to the Operational Lead, the role is responsible for embedding the service model principles across Senior Services streams, enabling the adopting of person-centred practices, promoting efficiency, and promoting opportunities for integration for service delivery.

Key responsibilities include:

  • Coach and guide the adoption of Uniting’s Senior Services models of care to improve service delivery outcomes.
  • Oversee local practice improvement initiatives in close collaboration with the local operational support team and service delivery teams to embed sustainability of practice change to improve customer outcomes.
  • Lead and role model effective program and process reviews to support the adoption of models of care that align to Senior Services Operating Model principles and person-centred practice.
  • Support the implementation of a ‘Continuum of Care’ model. 
  • Drive capability development and capacity building across services in the adoption of person-centred care principles/ models that promote positive outcomes.
  • Enable improvement initiatives to enhance the client and staff experience across dimensions of quality, cost, and service.
  • Consider and recommend initiatives that support Uniting strategies and the Inspired Care principles, approach, and business outcomes.

Qualifications, skills & experience

  • Typically, this role will require 5 or more years’ experience in your field of expertise.
  • Extensive coaching and/or change management experience using structured coaching/change management approaches.
  • Highly developed analytical skills, with the ability to manipulate large datasets, both quantitative and qualitative, and critically review same to inform decision making.

  • Postgraduate qualifications in Coaching and/or Change Management
  • Qualifications in Allied Health or Clinical (e.g., Nursing)
