About the role

We are seeking a dynamic, committed and capable leader for our Social Work team at Braeside Hospital and our South-Western Sydney community palliative care services. This leader will need to demonstrate a passion for their work, and a commitment to work within the Mission in Action of HammondCare. They will lead the planning, management and delivery of Social Work Services, to support the provision of skilled, effective and timely Social Work interventions to our clients and their carers. This includes continuous development of a high quality, evidence based social work service and skilled social work practitioners across the region, who convey the organisation’s Mission and Values, and operate consistently within the ethics and values of the AASW. They will participate as an active member of the HammondCare Health and Hospitals leadership team, reporting to the Director of Allied Health.

The Manager will support team members working within a multi-disciplinary context from all areas of our clinical specialties – palliative care, rehabilitation and older persons mental health – in inpatient, outpatient and community settings. This role supports staff working within a highly diverse cultural and linguistic environment, with SWSLHD having more than 55% of people speaking languages other than English at home. HammondCare’s focus is on enhancing the quality of life of our clients, with a holistic approach that respects diversity, and embraces physical, psychological, emotional, spiritual and practical issues. This role includes a client caseload in addition to the management responsibilities.

This position is based at Braeside Hospital in Prairiewood, with the capacity to travel across to other HammondCare sites as required. 

Responsibilities

  • Ensure the provision of a high quality, evidence based social work service, that maintains the values and standards of HammondCare, the AASW, and complies with all relevant policies, guidelines and legislative requirements.
  • Provide management, development, professional supervision and leadership to the Social Work team across all areas of clinical specialty, within a culture of continuous quality improvement. 
  • Maintain a part-time caseload, providing skilled, effective and timely social work services to clients and their carers, providing a holistic psychosocial perspective and collaborating as an effective member of the multi-disciplinary team. 
  • Maintain effective working relationships with internal and external stakeholders across SWSLHD, to facilitate the seamless transfer of clients between services, the interchange of information, and a streamlined and effective health care service.
  • Develop and implement Social Work Service Development Plans identifying and addressing trends, issues, service needs, team knowledge and skills development, best practice improvements etc.   
  • Implement and monitor new procedures, programs or policies in consultation with Social Work staff, Manager Social Work (north) and other Managers, based on evidence and current best practice.
  • Participate in service planning, implementation and monitoring within clinical units / services and across departments / other service portfolio areas, in collaboration with Director of Allied Health, senior nursing staff, Service Directors, and other Managers.
  • Ensure all internal and external compliance and reporting requirements relating to the provision of Social Work services are met.

Essential criteria

  • Recognised tertiary qualification in Social Work and eligibility for membership to AASW. Overseas qualifications must provide AASW Overseas Assessment number.
  • Current working rights in Australia i.e. citizenship, permanent residency or appropriate working visa.
  • Commitment to engage and align with HammondCare’s Mission in Action.
  • Minimum 5 years full-time post graduate social work experience with demonstrated knowledge and clinical skills providing a wide range of complex Social Work interventions within a health related service - preferably including palliative care / life limiting conditions, rehabilitation and/or mental health. Experience across inpatient, outpatient and community settings is desirable.
  • Demonstrated ability to provide support, guidance and leadership to a Social Work team, including providing professional / clinical supervision. Experience as Team Leader / Senior Social Worker is desirable.
  • Demonstrated management competencies including service planning and implementation, complex decision making, quality improvement, resource management, managing staff and leadership.
  • Demonstrated high level communication and interpersonal skills (written, verbal and electronic), across a diverse range of internal and external stakeholders and purposes. 
  • Effective skills in time management, organisation and managing competing demands.
  • Knowledge and understanding of the Health, Aged Care and NDIS sectors, including services / eligibility, policies / protocols, MOH guidelines, hospital accreditation and relevant legislation. Knowledge of NSW Health Policies / Guidelines is desirable.
  • Strong administrative and computer skills. Experience with NSW Health Electronic Medical Record or similar is desirable.
  • Skills, knowledge and experience working within a culturally and linguistically diverse environment.
  • Current unencumbered Drivers Licence and safe driving record. Willingness to travel across the region and work between HammondCare’s sites as required.
