The Department of Economic is seeking an experienced and highly motivated individual to join our team as a Case Manager 3. This position will provide service and support to individuals with developmental disabilities, and their families, acts as a lead worker in the unit and carries more complex and high-profile cases, and as a lead worker serves as a back-up to the supervisor.

Applicants for this position cannot be licensed or certified by the DDD Office of Licensing Certification and Regulation, cannot have a principal interest in or have spouse or close family with principal interest in a DDD Qualified Vendor Agreement, and cannot be employed or subcontracted by a DDD Qualified Vendor.

Job Duties:

Essential Duties and Responsibilities include but are not limited to:

  • Research community for available resources to meet needs identified in plan and to inform member of resource options
  • Facilitate meetings and program visits to assist with member/family choices
  • Review, monitor, and evaluate individual support plan in accordance with mandated timelines
  • Drive vehicle to conduct on-site visits to members/families home, schools, and program sites to discuss service and support needs
Knowledge, Skills & Abilities (KSAs):

Knowledge in:

  • Principles and practices of case management, applied behavioral interventions and child development
  • Methods, procedures, and techniques used in the development of individual support plans
  • Community resources, medical and behavioral health issues and terminology

Skills in:

  • Developing, evaluating, and maintaining member service plans
  • Interviewing, counseling, and interacting with members and family members
  • Conducting needs assessments, and problem identification with resolution

Abilities in:

  • Work independently and within diverse work teams
  • Maintain confidentiality
  • Lead a team of support coordinators in the absence of the supervisor

Pre-Employment Requirements:
  • An Arizona licensed registered nurse in good standing OR a degree in social work, OR a bachelor's degree in psychology, special education or counseling AND have at least one (1) year experience providing case management services to persons who are elderly and/or persons with physical or developmental disabilities and/or members determined to have a Serious Mental Illness (SMI) OR have a minimum of two (2) years’ experience providing case management services to persons who are elderly and/or persons with physical or developmental disabilities and/or members determined to have a Serious Mental Illness (SMI).
  • Successfully pass background and reference check; employment is contingent upon completion of the above mentioned process and the Agency’s ability to reasonably accommodate any restrictions.
  • Employee shall have a valid Level One fingerprint clearance card issued pursuant to Arizona Revised Statute 41-2758.07 in order to work with children and vulnerable adults.
  • Candidates for this position shall be subject to a search of both the Department of Child Safety Central Registry pursuant to A.R.S. 8-804 and the Adult Protective Services Registry pursuant to A.R.S. 46-459.
  • This position requires driving or the use of a vehicle as an essential function of the job to conduct State business, then the following requirements apply: Driver’s License Requirements.

Retirement:

State employees are required to participate in the Arizona State Retirement System (ASRS), the State sponsored retirement contribution plan and the Long-Term Disability (LTD) program after a 27-week waiting period. The ASRS defined benefit plan provides for life-long income upon retirement. You will also have the option to participate in a voluntary deferred compensation program to take advantage of tax-deferred retirement investments.
