Job Summary:

In collaboration with the multidisciplinary treatment team, oversees the development, review/revision, of assigned treatment plans, ensuring compliance with all licensing and regulatory bodies. Provides case management duties for the patients of the Arizona State Hospital.

Job Duties:

- Coordination of development, review, and revision of treatment plans in collaboration with the multidisciplinary treatment team, ensuring compliance with all licensing and regulatory body requirements for treatment plans.


- Provides notifications of outside agencies when requested by statue, rule, licensure or certification or policy, and completes associated documentation
.


- Assists in case management for coordination of patient admissions and discharges, to and from the hospital. This includes, but is not limited to the following: contacting outpatient providers, completing related hospital forms, and procuring releases of information.


- Provides department support according to level of education and credentials (i.e.: psycho-educational family group, case management, data collection, administrative tasks, education program).


- Other duties as assigned as related to the position (typically 5% - 10%)

Knowledge, Skills & Abilities (KSAs):

Knowledge of:

- Understanding of mental illness and basic understanding of current theories of mental
illness and treatment of persons with mental illness.
- Performance Management (PM), and Continuous Quality Improvement (CQI) and
Lean methodologies.

Skill in:

- Excellent verbal and written communication.
- Diplomatic and professional demeanor.
- Strong skill with computer programs in Microsoft office suite, and electronic medical
records.

Ability to:

- Work with a diverse patient population.
- Support a diverse multi-cultural workforce that reflects the community, promotes equal

Selective Preference(s):

Candidate will have a Bachelor's Degree in Psychology, Social Work, or related field.
Preference given to applicants with Public Health experience.

Pre-Employment Requirements:

A State Hospital employee must possess a valid level one fingerprint clearance card issues pursuant to A.R.S.41-1758.07 or must apply for a level one fingerprint clearance card within seven working days after beginning employment.

Retirement:

To help you build a financially secure future, the State makes monthly contributions to finance your retirement benefit. The State will make a contribution to the ASRS in an amount equal to your contribution. In other words, you and the State will each pay 50% of the total cost of the benefit. New State employees have a 27 week wait period for contributions to begin.
