The Department of Economic Security, Division of Developmental Disabilities (DDD), is seeking an experienced and highly motivated individual to join our team as a Hab Nurse 2. DDD provides individuals with developmental disabilities, and their families, services and supports that are flexible, high quality, and member driven. These services provide individuals with opportunities to exercise their rights and responsibilities of independent decision-making and engagement in the community.

This position provides specialized nursing care for individuals with developmental disabilities requiring regular nursing care; provides specialized medical support to staff; completes required charting, reporting, and recommended health care plans; develops health education plans in specialty areas; ensures physicians’ orders are obtained, updated, and renewed with therapeutic interventions; administers medications and provides skilled treatment; acts as a consultant to peers and staff regarding treatment of members’ illness, rehabilitation needs, and preventative care; monitors assigned site for compliance with Department of Health standards; participates as a member of interdisciplinary teams; conducts ongoing quality assurance monitoring.

Applicants for this position cannot be licensed or certified by the DDD Office of Licensing Certification and Regulation, cannot have a principal interest in or have a spouse or close family with principal interest in a DDD Qualified Vendor Agreement, and cannot be employed or subcontracted by a DDD Qualified Vendor.

Job Duties:

Essential Duties and Responsibilities include but are not limited to:

• Assesses health needs and completes a nursing assessment of members with developmental disabilities; makes nursing diagnosis and submits to designated departments.
• Participates in fact findings related medical issues; completes monitoring of medical care; monitors medical administration systems; provides feedback.
• Notes, orders and prepares reports for basic care, nursing assessments, and Person Centered Service Plan (PCSP). Completes utilization review on caseload and documents on-going medical needs; submits to designated department; discharge and admission planning.
• Examines and analyzes medical records and charts to ensure quality care.
• Participates in the development of policies and procedures and related problem solving. Develops and provides education classes for staff.

Knowledge, Skills & Abilities (KSAs):

Knowledge in:

• Nursing theory and practice, particularly as related to Developmental Disabilities; and Intermediate Care Facilities for Individuals with Intellectual Disabilities (ICF/ID) and State Licensing requirements
• State Nurse Practice Act and Title XIX federal regulations and licensing standards
• Agency and Division State-Operated Services policy and procedures
• Member condition and medical/physical changing care needs
• Interdisciplinary team planning process, services offered in community, including primary health care providers and educational material resources

Skills In:

• Nursing assessment (all skills allowed by scope of practice), making sound medical decisions and time management
• Providing appropriate treatment in emergency situations
• Interpersonal communication
• Communicating clearly and concisely in writing and verbally in addition to effective problem-solving
• Providing formal instruction and developing, analyzing, and implementing treatment care plans

Ability to:

• Listen, observe, assess, and respond to medical emergencies
• Make independent decisions regarding member health concerns
• Present training material in a clear and precise manner
• Respond to medical issues/inquires in a professional manner
• Work with professional and non-professional staff to ensure health and safety of residents

Selective Preference(s):

The ideal candidate for this position will have:

• At least 1 year experience in providing professional hands-on nursing services to individuals with developmental disabilities AND at least 1 year of experience in professional community nursing.

Pre-Employment Requirements:

• Required at time of appointment: possession of a current license to practice as a registered nurse in the State of Arizona.
• Employee shall have a valid Level One fingerprint clearance card issued pursuant to Arizona Revised Statute 41-2758.07 in order to work with children and vulnerable adults.
• Candidates for this position shall be subject to a search of both the Department of Child Safety Central Registry pursuant to A.R.S. 8-804 and the Adult Protective Services Registry pursuant to A.R.S. 46-459.
• Employment may be contingent upon completion of a post-offer medical/physical examination and the agency’s ability to reasonably accommodate any restrictions.
• If this position requires driving or the use of a vehicle as an essential function of the job to conduct State business, then the following requirements apply: Driver’s License Requirements. Any employee operating a motor vehicle on State business must possess and retain a current, valid class-appropriate driver's license, complete all required training, and successfully pass all necessary driver's license record checks. The license must be current, unexpired, and neither revoked nor suspended. Employees who drive on state business are subject to Driver's license record checks, must maintain acceptable driving records, and must complete all required driver training. An employee who operated a personally owned vehicle on state business shall maintain the statutorily required liability insurance (see Arizona Administrative Code (A.A.C.) (R2-10-201.11).
• Successfully pass background and reference check; employment is contingent upon completion of the above-mentioned process and the Agency's ability to reasonably accommodate any restrictions.

Retirement:

State employees are required to participate in the Arizona State Retirement System (ASRS), the State sponsored retirement contribution plan and the Long-Term Disability (LTD) program after a 27-week waiting period. The ASRS defined benefit plan provides for life-long income upon retirement. You will also have the option to participate in a voluntary deferred compensation program to take advantage of tax-deferred retirement investments.
