This position carries out all geriatric nursing care, treatment and services in a professional and prescribed manner within scope of license and provides a safe, therapeutic, and hygienic environment while overseeing the duties of subordinate staff.
The number of LPN and CNA positions assigned to work on a unit varies according to the shift.
The LPN learns, implements, and monitors Arizona Management System (AMS) methods, uses AMS concepts and tools for problem solving, continuous work/process improvements and efficiencies and incorporates a cultural focus on Veteran/Member-centered care in the performance of job duties.
Job Duties: |
As a Licensed Practical Nurse, you will:
- Administer medication and perform treatments on an assigned unit as ordered by the physician and within scope of license. (Medications may be in a variety of forms including oral, enteral, parenteral, suppository, etc. and treatments may include sterile and aseptic wound cleaning and dressing, postural drainage and percussion, tracheostomy, gastrostomy and jejunostomy care, and catheter care.)
- Observe and report the effectiveness of medications and treatments
- Ensure an adequate amount of supplies and medication are available
- Receive and deliver a variety of information regarding the resident's medical, social, functional, and other needs
- Participate in the receiving and giving shift reports
Communicate with residents, family, and staff as necessary to ensure the resident receives appropriate care from the interdisciplinary team - Complete the initial admission nursing assessment tool on a newly admitted resident
- Assist with the development of the Minimum Data Set (MDS) assessments, ongoing quarterly reviews and care plans
- Maintain appropriate clinical and administrative documentation regarding diagnosis, observations, treatment, and summary of resident progress.
- Participate in our Quality Assurance and Performance Improvement (QAPI) program
- Attend and participate in staff meetings
Knowledge, Skills & Abilities (KSAs): |
Knowledge of:
- Gerontological/Geriatric nursing theory and practice
- Effects of the aging process
- Practices and procedures of team nursing
- Practices and procedures for implementing treatment plans for resident/patient care
- Practices and procedures of pharmacology and medicines
- Federal, State and agency laws, rules and regulations governing general practical nursing practices and the security and control of medical supplies and medications and dispensing thereof
Skills in:
- Observing and reporting behavior and progress of residents/patients
- Applying and interpreting State and agency laws and regulations governing geriatric nursing
practices - Assisting in the development and implementation of patient care plans within a team nursing
concept - Oral and written communication skills
- Interpersonal skills
Ability to:
- Abide by nursing regulations
- Listen, observe, assess and respond to medical emergencies
- Evaluate, analyze, make independent decisions and follow up regarding client health problems
- Respond to medical issues/inquiries in a professional manner

Selective Preference(s): |
On or more years of work experience in a Skilled Nursing/ Long-term Care facility
Pre-Employment Requirements: |
- Must possess an active license to practice as a Licensed Practical Nurse in the State of Arizona
- Must be able to obtain and retain a fingerprint clearance card issued by the Arizona Department
of Public Safety - May be required to drive on State duty, valid state-issued driver's license is required
- Current Negative Tb skin test (Mantoux skin test) within the last 6 months or a written statement from a physician, physician’s assistant or a registered nurse practitioner indicating freedom from tuberculosis, if in the past has had a positive skin test or tuberculosis
