Job Summary: |
The purpose of this position is to ensure resident care is provided per the service plan. Care plan coordination includes heading the IDT meeting, scheduling IDT meetings and informing responsible parties and ensuring service plans are up to date and in compliance with DHS regulations.
The position is responsible to provide case management oversight of residents. This includes the management of on-site clinic visits with the physician or accompanying residents to consult visits as indicated.
Responsibilities include supervision of subordinate staff, supervising a shift of para-,pre- and professional nursing staff who provide care and treatment for geriatric residents. Performs nursing duties as needed including filling in a shift.
Ensures adequate documentation of resident care. Ensures designated staff are adequately trained. Assists in developing and conducting in-service training programs, continuing education and staff orientation.
Ensures staff care in a safe manner for themselves and residents. Ensures analysis of incidents and infections, monitoring for the trends requiring improvements. Participates in after hours on call schedule.
Job Duties: |
1) Nursing Staff Supervision: Supervises, guides, counsels, and instructs nursing staff consisting of assistant caregivers, certified caregivers, certified nursing assistance (CNAs), ward clerk, driver, licensed practical nurses (LPNs) and registered nurses (RNs). Performs on-site corrective action and writes up Memos of Concern (MOCs) for those staff and completes any further disciplinary action in consultation with the Director of Nursing (DON). Participates in annual evaluations and formal counseling with DON. Participates in selection of new employee hires.
2) Nursing Services: Provides case management oversight to residents. Performs admission and readmission of residents. Manages on site clinic visits and goes with resident to off site consulting physician visits if indicated. Attends nursing management meetings.
3) Patient Care: Oversees and performs direct resident care such as medication administration, wound and treatment care, injections, lab draws, catheterizations and nursing assessments as needed. Creates and implements appropriate resident care plans. Daily checks on residents with Change of Condition and those in sick rooms then reviews concerns with the DON.
4) Staff Development: Attends and provides input into monthly nurses and nursing assistant/caregiver meetings; assist/provide new nursing staff orientation; provide and track continuing education/in-services, performs nursing staff skills/competency assessments and reviews.
5) Quality Assurance: Assists with tracking and administering resident TB testing, resident influenza and pneumonia vaccinations. Assist with tracking, monitoring and analysis of infection/incident reports. Monitors and reviews nursing documentation such as monthly summaries, care plan summaries, COC care plans, MARS/TARS and noting of orders. Participates in nursing QA/QI initiatives.
6) Care Planning Coordination: Care plan coordination includes heading the IDT meeting, scheduling DT meetings and informing responsible parties and ensuring service plans are up to date and in compliance with DHS regulations. The position is responsible to provide case management oversight of residents.

Knowledge, Skills & Abilities (KSAs): |
Knowledge:
• Knowledge of LT care nursing department operations, nursing management practices, advanced clinical assessment and disease management practices as well as Federal, State and local laws that govern LT care facilities and apply to nursing practices and protocols
• Knowledge of federal, state and agency laws, rules and regulations governing the control, security and dispensing of medical supplies and medication
• Knowledge of the principles, practices and techniques of professional geriatric nursing care, treatment and services for geriatric residents
• Knowledge of federal, state and agency rules, regulations and procedures governing geriatric nursing services
• Knowledge of practices and techniques of providing a safe, orderly and therapeutic environment
• Knowledge of long term care, with a minimum of 5 years long term care experience
• Knowledge of practices and techniques of work leadership and management
• Knowledge of resident assessments and protocols and tools
Skills:
• Skill to interpret Federal, State, nursing associations and agency laws, rules and regulations governing geriatric nursing services
• Skill or ability to instruct in and perform geriatric nursing care, treatment and services, care for the infirm and apply
• Demonstrate excellent analytical, clinical, problem-solving, judgment and decision making skills
• Skill to establish and maintain working relationships specific to the work setting
• Must be able to read, write, speak and understand the English language
• Skill to provide a safe, orderly and therapeutic environment, as above
• Skill in using Microsoft Word, Computer based Care Plan Software
• Skill to communicate well both verbally and in writing
• Must be skilled in time management and multi-tasking
Ability:
• Must possess a professional demeanor and exhibit the ability to develop excellent relations with residents, families, visitors, facility personnel and agency management staff
• Ability to develop, implement and maintain nursing clinical and management systems to effect best nursing practices and care outcomes
• Ability to have patience, tact and a maturity level with which to interact with people at whatever mental functioning level
• Planning and organizing abilities to effect quick and ordered resolution to identified areas needing improvement
• Ability to read, write and speak the English language
Selective Preference(s): |
Required:
• The individual must possess a current Arizona Registered Nursing License Current CPR
Preferred:
• Extensive experience in LTC/Geriatrics with an understanding of case management and care planning in LTC.
Pre-Employment Requirements: |
• Must undergo annual screening for TB with negative results
• Valid Arizona Class 1 fingerprint card within 20 days of hire
